INDICATION
Haemaccel Infusion is a colloid plasma volume expander used for emergency restoration of circulating blood volume in patients experiencing hypovolemic shock from hemorrhage, burns, trauma, or major surgery. It is administered when crystalloid solutions alone cannot maintain adequate blood pressure and tissue perfusion, and when blood products are unavailable or not yet crossmatched.
MECHANISM OF ACTION
Polygeline is a macromolecular colloid that generates oncotic pressure within the intravascular space. When infused intravenously, it draws interstitial fluid into the bloodstream by osmotic gradient, rapidly expanding plasma volume and improving cardiac output, blood pressure, and organ perfusion. Polygeline has a molecular weight of approximately 30,000 Daltons and a plasma half-life of 3 to 6 hours. The solution also contains calcium chloride, sodium chloride, and potassium chloride in physiological concentrations.
DOSAGE AND ADMINISTRATION
● Administered intravenously under physician supervision in a hospital or emergency setting only
● Dose and infusion rate depend on the severity of shock and the patient's hemodynamic response
● In acute hemorrhagic shock: rapid infusion of 500ml to 1500ml may be required initially
● Monitor blood pressure, pulse, urine output, and central venous pressure continuously during infusion
● Maximum recommended volume: 1500 to 2000ml unless clinical necessity demands more
● Warm the solution to body temperature before rapid large-volume infusion to prevent hypothermia
SIDE EFFECTS
Common:
● Mild urticaria or skin flushing due to histamine release from the gelatin component
● Transient tachycardia during rapid infusion
Less common:
● Anaphylactoid reactions presenting with bronchospasm and hypotension in rare cases
● Dilutional coagulopathy with large-volume administration
● Fluid overload in patients with impaired cardiac or renal function
NOT RECOMMENDED IN
● Known hypersensitivity to gelatin-based colloid solutions
● Hypervolemia or established severe cardiac failure
● Severe pre-existing coagulation disorders
● Late pregnancy due to anaphylactoid risk to the fetus
DRUG INTERACTIONS
Polygeline may potentiate the anticoagulant effect of warfarin or heparin through dilutional coagulopathy at large volumes. Calcium content in the solution may interact with digitalis glycosides, increasing the risk of cardiac toxicity. Do not mix with blood products in the same infusion line. Verify drug compatibility before adding any medication to the infusion.
STORAGE INSTRUCTIONS
