INDICATION

Gvia-M XR 50/500mg Tab is an extended-release combination of sitagliptin and metformin for type 2 diabetes management. The XR formulation allows once-daily dosing with reduced gastrointestinal side effects compared to immediate-release metformin, improving patient adherence.

MECHANISM OF ACTION

Same as other sitagliptin-metformin combinations. The extended-release metformin matrix releases metformin gradually over 12 to 24 hours, reducing peak plasma concentrations and smoothing absorption — which is the main reason for metformin's GI side effects. Sitagliptin is released in a conventional manner.

DOSAGE AND ADMINISTRATION

•      1 to 2 tablets once daily with the evening meal

•      Extended-release tablets must be swallowed whole — do not crush or chew

•      Take with food to optimize metformin absorption

•      Monitor renal function

SIDE EFFECTS

Common:

•      Diarrhoea, nausea (less frequent than with IR metformin)

•      Nasopharyngitis

•      Headache

Less common:

•      Pancreatitis

•      Lactic acidosis (rare)

•      Hypoglycemia with insulin or sulphonylureas

NOT RECOMMENDED IN

•      eGFR below 30ml/min

•      Type 1 diabetes or DKA

•      Hepatic impairment

DRUG INTERACTIONS

Sitagliptin with insulin or sulphonylureas increases hypoglycemia risk; consider dose reduction. Hold metformin 48 hours before iodinated contrast procedures. Avoid alcohol as it raises lactic acidosis risk. ACE inhibitors may slightly increase angioedema risk. Sitagliptin has no significant CYP450 interactions.
