INDICATION

Gvia-M 50/500mg Tab is used for type 2 diabetes management when both sitagliptin and the lower 500mg metformin dose are required. It is suitable for patients initiating combination therapy or those who cannot tolerate higher metformin doses due to gastrointestinal side effects.

MECHANISM OF ACTION

Same dual mechanism as Gvia-M 50/1000mg. Sitagliptin enhances incretin-mediated glucose-dependent insulin secretion. Metformin 500mg provides moderate hepatic glucose suppression and insulin sensitization with lower GI side effects than the 1000mg dose.

DOSAGE AND ADMINISTRATION

•      1 tablet twice daily with meals

•      Start at this dose and titrate to higher metformin strengths based on tolerability

•      Monitor eGFR regularly

SIDE EFFECTS

Common:

•      Nausea, diarrhoea (generally milder than higher metformin doses)

•      Nasopharyngitis

•      Headache

Less common:

•      Pancreatitis

•      Lactic acidosis (rare)

•      Joint pain

NOT RECOMMENDED IN

•      eGFR below 30ml/min

•      Type 1 diabetes or DKA

•      Hepatic impairment

DRUG INTERACTIONS

Gvia M may interact with other antidiabetic drugs such as Insulin, increasing the risk of hypoglycemia. It can also interact with kidney-affecting drugs like Furosemide, which may raise metformin levels. Corticosteroids such as Prednisolone may increase blood glucose levels and reduce the effectiveness of Gvia M.
