INDICATION

Gvia-M 50/1000mg Tab is a fixed-dose combination of sitagliptin and metformin for type 2 diabetes management in adults requiring both a DPP-4 inhibitor and higher-dose metformin. It is used when sitagliptin monotherapy does not provide adequate glycemic control in patients already tolerating metformin 1000mg twice daily.

MECHANISM OF ACTION

Sitagliptin inhibits DPP-4, increasing active GLP-1 and GIP incretin levels and improving glucose-dependent insulin secretion and glucagon suppression. Metformin reduces hepatic glucose production and improves insulin sensitivity. The 1000mg metformin dose provides robust hepatic glucose suppression. Together they provide synergistic glycemic control with a low risk of hypoglycemia.

DOSAGE AND ADMINISTRATION

•      1 tablet twice daily with meals

•      For patients requiring high-dose metformin

•      Dose reduction required in moderate renal impairment

•      Monitor eGFR — do not use if below 30ml/min

SIDE EFFECTS

Common:

•      Nausea, diarrhoea from high-dose metformin

•      Nasopharyngitis

•      Headache

Less common:

•      Pancreatitis

•      Lactic acidosis (rare)

•      Bullous pemphigoid

NOT RECOMMENDED IN

•      eGFR below 30ml/min

•      Type 1 diabetes or DKA

•      Hepatic impairment

•      History of pancreatitis

DRUG INTERACTIONS

Sulphonylureas and insulin increase hypoglycemia risk. Alcohol increases lactic acidosis risk.
