INDICATION
Gvia 50mg Tab is a DPP-4 inhibitor used as an adjunct to diet and exercise for the management of type 2 diabetes mellitus in adults. It is used as monotherapy when metformin is not tolerated, or in combination with other antidiabetic agents including metformin, sulphonylureas, thiazolidinediones, and insulin. The 50mg dose is used when combined with a sulphonylureas or insulin, where dose reduction is required to minimize hypoglycemia.
MECHANISM OF ACTION
Sitagliptin inhibits dipeptidyl peptidase-4 (DPP-4), the enzyme that rapidly inactivates incretin hormones including GLP-1 and GIP. By inhibiting DPP-4, sitagliptin increases active incretin levels, which stimulate glucose-dependent insulin secretion from pancreatic beta cells and suppresses glucagon from alpha cells. This mechanism improves both fasting and post-prandial glycemic control with a low intrinsic risk of hypoglycemia.
DOSAGE AND ADMINISTRATION
• 100mg once daily (standard dose) — the 50mg dose is typically used as part of combination therapy where dose adjustment is needed
• When combined with sulphonylureas or insulin: 50mg once daily to reduce hypoglycemia risk
• May be taken with or without food
• Dose reduction required in moderate to severe renal impairment
SIDE EFFECTS
Common:
• Nasopharyngitis, upper respiratory tract infections
• Headache
• Nausea
• Hypoglycemia when combined with sulphonylureas or insulin
Less common:
• Pancreatitis (rare but serious)
• Bullous pemphigoid (rare skin blistering)
• Joint pain (arthralgia)
• Hypersensitivity reactions including angioedema
NOT RECOMMENDED IN
• Type 1 diabetes or diabetic ketoacidosis
• Known hypersensitivity to sitagliptin
• History of pancreatitis (use with caution)
• Severe renal impairment — adjust dose
DRUG INTERACTIONS
Sulphonylureas and insulin increase hypoglycemia risk when combined with sitagliptin; consider reducing their dose. ACE inhibitors combined with DPP-4 inhibitors may increase the risk of angioedema. Sitagliptin does not significantly inhibit or induce cytochrome P450 enzymes.
