INDICATION

GTN Paste 30GM is a compounded glyceryl trinitrate preparation prescribed primarily for the treatment of chronic anal fissures that have not responded to conservative management. It relaxes the internal anal sphincter, restores blood flow to the fissure bed, and promotes tissue healing. It is also used in select cases for angina prophylaxis when applied topically to the chest.

MECHANISM OF ACTION

Glyceryl trinitrate releases nitric oxide upon tissue contact. Nitric oxide activates guanylate cyclase in vascular smooth muscle, raising cyclic GMP levels and causing smooth muscle relaxation. In the anal canal, this lowers resting sphincter pressure, improves local perfusion, and allows the ischemic fissure to heal. On the chest wall, venodilation reduces cardiac preload and myocardial oxygen demand, preventing anginal episodes.

DOSAGE AND ADMINISTRATION

●     For anal fissure: apply a pea-sized amount to the internal and external anal margin every 12 hours using a finger cot or glove

●     Treatment duration is typically 6 to 8 weeks

●     For angina prophylaxis: apply as directed by the prescribing physician

●     Wash hands thoroughly after each application

●     Rotate application sites if used on the chest to prevent skin irritation

●     Do not apply to broken or severely inflamed skin

SIDE EFFECTS

Common:

●     Headache, often throbbing, which usually improves with continued use

●     Facial flushing

●     Dizziness or lightheadedness on standing

Less common:

●     Postural hypotension or fainting with excessive application

●     Tachycardia or palpitations

●     Perianal irritation or contact dermatitis at the application site

●     Tolerance development with uninterrupted continuous use

NOT RECOMMENDED IN

●     Concurrent use of phosphodiesterase-5 inhibitors such as sildenafil, tadalafil, or vardenafil is absolutely contraindicated due to risk of severe, potentially fatal hypotension

●     Severe hypotension or hypovolemia

●     Hypertrophic obstructive cardiomyopathy or constrictive pericarditis

●     Raised intracranial pressure

●     Known hypersensitivity to nitrates or nitrites

DRUG INTERACTIONS

Concurrent use with sildenafil, tadalafil, or vardenafil causes potentially life-threatening hypotension and is absolutely contraindicated. Other antihypertensives and vasodilators may have additive blood pressure lowering effects. Alcohol enhances the hypotensive action. Ergotamine may antagonize the vasodilating effect.
