INDICATION
Genticyn 80mg Injection is indicated for the treatment of serious systemic bacterial infections caused by susceptible gram-negative organisms including septicemia, urinary tract infections, respiratory tract infections, bone and joint infections, central nervous system infections, intra-abdominal infections, and skin and soft tissue infections where oral therapy is not feasible.
MECHANISM OF ACTION
Gentamicin is a bactericidal aminoglycoside antibiotic that irreversibly binds to the 30S ribosomal subunit of bacteria, causing misreading of mRNA and production of abnormal, non-functional proteins. This leads to disruption of the bacterial cell membrane and cell death. Gentamicin is concentration-dependent in its killing activity and exhibits a significant post-antibiotic effect, allowing extended-interval dosing. It is particularly effective against Pseudomonas aeruginosa, E. coli, Klebsiella, and Serratia species.
DOSAGE AND ADMINISTRATION
● Administered intramuscularly or intravenously under medical supervision
● Standard adult dose: 3 to 5mg/kg/day in divided doses every 8 hours or as a single daily dose
● Pediatric dose: 2.5mg/kg every 8 hours or as prescribed
● Duration is typically 7 to 10 days
● Dose adjustment is essential in renal impairment based on creatinine clearance
● Monitor serum drug levels (trough and peak) to ensure therapeutic range and avoid toxicity
SIDE EFFECTS
Common:
● Nephrotoxicity with prolonged use or high doses
● Ototoxicity affecting hearing or balance
● Injection site pain
Less common:
● Neuromuscular blockade, particularly in patients with myasthenia gravis
● Peripheral neuropathy
● Elevated liver enzymes
NOT RECOMMENDED IN
● Known hypersensitivity to gentamicin or other aminoglycosides
● Myasthenia gravis
● Concurrent use with other ototoxic or nephrotoxic agents without monitoring
● Pregnancy unless benefits clearly outweigh risks
DRUG INTERACTIONS
Loop diuretics such as furosemide may potentiate ototoxicity and nephrotoxicity. Other nephrotoxic drugs including vancomycin, amphotericin B, and ciclosporin increase kidney damage risk. Neuromuscular blocking agents may have enhanced paralysis with concurrent gentamicin. Penicillins may inactivate gentamicin if mixed in the same IV bag.
