INDICATION
Gen-M 60mg Inj is an injectable artesunate formulation used for the treatment of severe and complicated Plasmodium falciparum malaria in patients who cannot take oral therapy. The 60mg dose is commonly used for weight-based dosing in pediatric patients and low body weight adults. Artesunate injection is the WHO-recommended first-line treatment for severe malaria, having demonstrated significantly lower mortality than quinine in large clinical trials.
MECHANISM OF ACTION
Same as Gen-M 120mg Injection. Artesunate is rapidly hydrolyzed to dihydroartemisinin (DHA), which generates reactive oxygen species inside malaria-infected erythrocytes, causing rapid killing of all erythrocytic stages including early ring forms that are sequestered in the microvasculature. This ring-stage activity is a key advantage over quinine and explains artesunate's superior efficacy in reducing mortality from severe malaria. The 60mg dose provides the standard 2.4mg/kg loading dose for patients weighing approximately 25kg.
DOSAGE AND ADMINISTRATION
• Severe malaria: 2.4mg/kg IV or IM at 0, 12, and 24 hours, then once daily
• 60mg loading dose for patients weighing approximately 25kg
• Children under 20kg: use 3mg/kg per dose
• Administer into the anterior thigh (IM) or slowly IV over 1 to 2 minutes
• Minimum 3 parenteral doses before switching to oral ACT
• Switch to oral antimalarial therapy as soon as the patient can tolerate oral medications
SIDE EFFECTS
Common:
• Post-artesunate delayed hemolysis (1 to 4 weeks after treatment)
• Injection site pain
• Nausea, vomiting
• Dizziness, headache
Less common:
• Transient neutropenia
• Elevated liver enzymes
• Reticulocytopenia
• Hypersensitivity reactions (rare)
NOT RECOMMENDED IN
• Hypersensitivity to artesunate or artemisinin derivatives
• Not for prophylaxis: only for treatment of active severe malaria
• First trimester pregnancy: use only if life-threatening and no safer alternative
DRUG INTERACTIONS
Fewer drug interactions than quinine. Avoid mefloquine within 24 hours. Use caution with other QT-prolonging agents. Rifampicin may reduce DHA levels.
