INDICATION

Gen-M 80/480mg Tab is an artemisinin-based combination therapy (ACT) used for the treatment of uncomplicated Plasmodium falciparum malaria in adults and adolescents weighing 35kg or more. WHO recommends ACTs as first-line treatment for uncomplicated falciparum malaria. This combination provides rapid parasite clearance through artemether and long-lasting cure through lumefantrine.

MECHANISM OF ACTION

Artemether is a methyl ether derivative of artemisinin. Inside malaria-infected red blood cells, it is activated by iron released from haem during hemoglobin digestion. The activated artemether generates reactive oxygen species that alkylate and damage key malaria parasite proteins, disrupting electron transport and causing rapid parasite death. It acts on all stages of the erythrocytic cycle and has rapid gametocytocidal activity, reducing malaria transmission. Lumefantrine is a long-acting antimalarial that inhibits the conversion of haem to hemozoin in the parasite food vacuole, causing toxic haem accumulation and oxidative parasite death. The combination targets two different mechanisms, reducing the likelihood of resistance development and ensuring clearance of residual parasites after artemether's rapid action.

DOSAGE AND ADMINISTRATION

      Adults and adolescents (above 35kg): 4 tablets at time 0, then 4 tablets at 8 hours, then 4 tablets twice daily for the following 2 days (total 24 tablets over 3 days)

      Take with food or a fatty drink to maximize lumefantrine absorption

      Do not take on an empty stomach as lumefantrine absorption is severely reduced

      Complete the full 3-day course even if symptoms improve

SIDE EFFECTS

Common:

      Headache, dizziness

      Nausea, vomiting

      Sleep disturbances

      Abdominal pain, anorexia

Less common:

      QT prolongation (monitor ECG in patients with cardiac risk factors)

      Palpitations

      Hemolysis in G6PD-deficient patients (delayed, 2 to 4 weeks after treatment)

NOT RECOMMENDED IN

      Known hypersensitivity to artemether or lumefantrine

      History of cardiac arrhythmia or QTc prolongation

      First trimester of pregnancy (use alternative if possible)

      Co-administration with other QT-prolonging drugs

DRUG INTERACTIONS

Strong CYP3A4 inhibitors (ketoconazole) increase lumefantrine levels. CYP3A4 inducers (rifampicin, efavirenz, nevirapine) reduce lumefantrine levels. QT-prolonging drugs increase cardiac risk. Halofantrine and mefloquine should not be used within 4 weeks of artemether-lumefantrine.

STORAGE INSTRUCTIONS

Store Gen-M 80/480mg tablets at room temperature below 30°C. Protect from moisture. Keep in original blister packaging. Store out of reach of children.
