INDICATION

Gen-M 120mg Injection is an injectable antimalarial containing artesunate, used for the treatment of severe and complicated malaria including cerebral malaria, severe malarial anemia, and malaria with multiorgan involvement. Artesunate injection is the WHO-recommended first-line treatment for severe falciparum malaria in both adults and children, as it has been shown to reduce mortality compared to quinine.

MECHANISM OF ACTION

Artesunate is a water-soluble hemi succinate derivative of artemisinin. After injection, it is rapidly hydrolyzed to dihydroartemisinin (DHA), the primary active metabolite. DHA generates reactive oxygen species through activation by iron released from hemoglobin digestion inside malaria-infected red blood cells. These free radicals alkylate and damage critical parasite proteins and membranes, producing rapid and potent killing of all erythrocytic stages of Plasmodium falciparum, including early ring-stage parasites that are sequestered in the microvasculature. This early ring-stage activity is a major advantage of artesunate over quinine and is the reason for its superior efficacy in severe malaria. Artesunate also has gametocytocidal activity, reducing malaria transmission.

DOSAGE AND ADMINISTRATION

      Severe malaria (adults and children): 2.4mg/kg IV or IM at 0, 12, and 24 hours, then once daily until patient can take oral medication

      Minimum 3 parenteral doses (24 hours) before switching to oral ACT to complete full treatment

      For children under 20kg: dose is 3mg/kg per dose due to higher drug clearance

      Reconstitute with sodium bicarbonate 5%, then dilute with normal saline or 5% dextrose for IV injection

      Administer IV slowly over 1 to 2 minutes or IM into the anterior thigh

      Switch to oral artemether-lumefantrine or other ACT once the patient can swallow reliably

SIDE EFFECTS

Common:

      Post-artesunate delayed hemolysis (PADH): occurs 1 to 4 weeks after treatment, especially in non-immune travelers and hyperparasitaemic patients

      Nausea, vomiting, diarrhoea

      Dizziness, headache

      Injection site pain

Less common:

      Neutropenia (transient)

      Elevated liver enzymes

      QT prolongation (less common than with quinine)

      Reticulocytopenia during the first week of treatment

      Hypersensitivity reactions (rare)

NOT RECOMMENDED IN

      Known hypersensitivity to artesunate or any artemisinin derivative

      First trimester of pregnancy: use only if the malaria is life-threatening and no safer alternative exists

      Not for malaria prophylaxis: only for treatment of active severe malaria

DRUG INTERACTIONS

Artesunate has fewer drug interactions than quinine. Co-administration with other QT-prolonging drugs should be done with caution. Mefloquine should not be given within 24 hours of artesunate to reduce neurotoxicity risk. Rifampicin and other strong enzyme inducers may reduce DHA exposure. Avoid concurrent nephrotoxic agents as severe malaria itself may impair renal function.
