INDICATION

Galza-Met 50/500mg Tab is a fixed-dose combination of a DPP-4 inhibitor and a biguanide used as an adjunct to diet and exercise in adults with type 2 diabetes mellitus who are not adequately controlled on metformin alone. It offers glucose-lowering with a low risk of hypoglycemia and is weight-neutral.

MECHANISM OF ACTION

Vildagliptin inhibits dipeptidyl peptidase-4 (DPP-4), the enzyme responsible for the rapid degradation of glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic peptide (GIP). By inhibiting DPP-4, vildagliptin increases and prolongs active incretin levels, which stimulate glucose-dependent insulin release from pancreatic beta cells and suppress glucagon secretion from alpha cells. Metformin reduces hepatic glucose production via AMPK activation and improves peripheral insulin sensitivity.

DOSAGE AND ADMINISTRATION

•      1 tablet twice daily with meals

•      Adjust based on patient's prior antidiabetic regimen

•      Monitor liver enzymes before and during therapy

•      Swallow tablets whole with water

•      Not recommended when eGFR is below 60ml/min/1.73m2 due to metformin component

SIDE EFFECTS

Common:

•      Nausea, diarrhoea from metformin

•      Headache, dizziness from vildagliptin

•      Nasopharyngitis

•      Constipation

Less common:

•      Hepatotoxicity — monitor liver function with vildagliptin

•      Pancreatitis (rare)

•      Peripheral oedema

•      Lactic acidosis from metformin (rare)

NOT RECOMMENDED IN

•      eGFR below 60ml/min/1.73m2

•      Hepatic impairment

•      Pregnancy

•      Diabetic ketoacidosis

•      History of pancreatitis

DRUG INTERACTIONS

Insulin and sulphonylureas increase hypoglycemia risk. Alcohol increases lactic acidosis risk from metformin. Iodinated contrast procedures — hold metformin 48 hours before. ACE inhibitors may increase risk of angioedema with DPP-4 inhibitors.
