INDICATION

Gabica CR 82.5mg is indicated for the management of neuropathic pain associated with diabetic peripheral neuropathy, postherpetic neuralgia, and fibromyalgia. It is also used as adjunctive therapy for partial-onset seizures in adults with epilepsy and for the treatment of generalized anxiety disorder.

MECHANISM OF ACTION

Pregabalin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding reduces calcium influx at nerve terminals, decreasing the release of excitatory neurotransmitters including glutamate, norepinephrine, and substance P. The controlled-release formulation provides steady-state plasma levels over 24 hours, improving tolerability and allowing once-daily dosing compared to immediate-release pregabalin.

DOSAGE AND ADMINISTRATION

●     Adults: 82.5mg once daily, preferably in the evening after a meal

●     Dose may be increased to 165mg once daily after 1 to 2 weeks based on response and tolerability

●     Swallow whole; do not crush, chew, or split the tablet

●     Dose adjustment required in renal impairment

●     Do not stop abruptly; taper gradually over at least one week

SIDE EFFECTS

Common:

●     Dizziness or drowsiness

●     Weight gain

●     Peripheral edema

Less common:

●     Blurred vision

●     Difficulty concentrating or memory impairment

●     Euphoria or mood changes

●     Withdrawal symptoms if discontinued abruptly

NOT RECOMMENDED IN

●     Known hypersensitivity to pregabalin

●     Severe renal impairment without dose adjustment

●     Caution in patients with a history of substance abuse

●     Not recommended in children and adolescents below 18 years

DRUG INTERACTIONS

CNS depressants including benzodiazepines, opioids, and alcohol may have additive sedative effects. Pregabalin may potentiate the effects of ethanol and lorazepam. ACE inhibitors may have increased risk of angioedema when combined with pregabalin. Thiazolidinediones combined with pregabalin may increase the risk of peripheral edema and weight gain.
