INDICATION

Foster NEXThaler 200/6 is indicated for regular maintenance treatment of moderate to severe asthma in adults requiring higher-dose inhaled corticosteroid with a long-acting bronchodilator. It is also used in the treatment of COPD with recurrent exacerbations in patients requiring both anti-inflammatory and bronchodilator therapy.

MECHANISM OF ACTION

This higher-strength formulation delivers 200mcg of beclomethasone dipropionate along with 6mcg of formoterol per actuation via the NEXThaler dry powder device. The increased corticosteroid dose provides stronger anti-inflammatory control for patients with moderate to severe disease. Beclomethasone suppresses airway inflammation at the gene transcription level, while formoterol provides sustained bronchodilation for up to 12 hours. The extra fine particle technology achieves deeper lung deposition for enhanced efficacy in small airways.

DOSAGE AND ADMINISTRATION

•      Adults: 2 inhalations twice daily is the standard dose for this strength

•      Titrate to the lowest effective dose after control is achieved

•      Inhale deeply through the breath-actuated device

•      Do not exceed the prescribed number of inhalations

•      Rinse the mouth with water after each use

•      Not to be used as a rescue inhaler

SIDE EFFECTS

Common:

•      Oral candidiasis

•      Headache

•      Tremor, muscle cramps, or palpitations

Less common:

•      Dysphonia or hoarseness

•      Adrenal suppression with prolonged high-dose use

•      Paradoxical bronchospasm in rare cases

•      Hyperglycemia or reduced bone mineral density with long-term high-dose corticosteroid exposure

•      Not for relief of acute bronchospasm

•      Children and adolescents below 18 years for this strength

DRUG INTERACTIONS

Same interactions as Foster 200/6mcg pMDI and Foster NEXThaler 100/6. Beta-blockers reduce bronchodilator efficacy. CYP3A4 inhibitors increase systemic steroid exposure. Caution with QTc-prolonging agents.
