INDICATION

Foster 200/6mcg Inhaler is indicated for the regular treatment of asthma in adults and adolescents where a combination of an inhaled corticosteroid (ICS) and a long-acting beta-2 agonist (LABA) is appropriate. It is also indicated for symptomatic treatment of COPD in patients with a history of exacerbations requiring regular bronchodilator and anti-inflammatory therapy.

MECHANISM OF ACTION

Beclomethasone dipropionate is an ICS prodrug that is activated by esterases in the lungs to its active metabolite beclometasone-17-monopropionate. It reduces airway inflammation by inhibiting inflammatory gene transcription, decreasing eosinophilic infiltration, and reducing mucus secretion and airway hyperresponsiveness. Formoterol is a selective long-acting beta-2 adrenergic agonist that relaxes airway smooth muscle by stimulating intracellular adenylyl cyclase, increasing cAMP levels, and activating protein kinase A. The combination provides both anti-inflammatory and sustained bronchodilatory effects.

DOSAGE AND ADMINISTRATION

•      Adults and adolescents above 12 years: 1 to 2 inhalations twice daily

•      Maximum: 2 inhalations twice daily (total 4 inhalations per day)

•      Shake the inhaler well before each use

•      Rinse the mouth with water after inhalation to reduce oral candidiasis risk

•      Prime the inhaler before first use or if not used for more than 14 days

•      Do not exceed the prescribed dose

 

SIDE EFFECTS

Common:

 

•      Oral candidiasis (thrush) due to the corticosteroid component

•      Headache

•      Tremor or palpitations from the LABA component

 

Less common:

 

•      Hoarseness or dysphonia

•      Paradoxical bronchospasm in rare cases

•      Tachycardia or QTc prolongation at high doses

•      Adrenal suppression with prolonged high-dose use

 

NOT RECOMMENDED IN

 

•      Not for relief of acute asthma symptoms (use a short-acting bronchodilator for rescue)

•      Patients with known hypersensitivity to beclomethasone, formoterol, or any excipient

•      Children below 12 years of age

DRUG INTERACTIONS

Beta-blockers including eye drops may diminish the bronchodilator effect of formoterol. Concurrent use with other beta-2 agonists may increase cardiovascular side effects. QTc-prolonging drugs such as certain antihistamines, macrolides, and antiarrhythmics may increase arrhythmia risk. CYP3A4 inhibitors such as ritonavir may increase systemic beclomethasone exposure.
