INDICATION

Fortum 500mg Injection is a third-generation cephalosporin antibiotic indicated for the treatment of serious bacterial infections caused by susceptible gram-negative organisms including Pseudomonas aeruginosa. It is used for lower respiratory tract infections, urinary tract infections, skin and soft tissue infections, bone and joint infections, intra-abdominal infections, central nervous system infections including meningitis, and septicemia.

MECHANISM OF ACTION

Ceftazidime exerts bactericidal activity by binding to penicillin-binding proteins (PBPs) on the inner membrane of bacterial cell walls, inhibiting the transpeptidation step of peptidoglycan synthesis. This disrupts cell wall integrity, causing osmotic instability and bacterial cell lysis. Ceftazidime is highly stable against many beta-lactamases and has a particularly strong activity against Pseudomonas aeruginosa, making it a preferred choice for Pseudomonal infections.

DOSAGE AND ADMINISTRATION

•      Adults: 1g to 6g daily, given intravenously or intramuscularly in divided doses every 8 to 12 hours depending on severity

•      For Pseudomonal infections or meningitis: 2g every 8 hours intravenously

•      Children above 2 months: 30 to 100mg/kg/day in 2 to 3 divided doses, maximum 6g/day

•      Neonates: 25 to 60mg/kg/day in 2 divided doses

•      Reconstitute with water for injection or compatible diluent before administration

•      Dose adjustment required in renal impairment based on creatinine clearance

 

SIDE EFFECTS

Common:

•      Diarrhea or loose stools

•      Injection site reactions including pain, phlebitis, or inflammation

•      Nausea or vomiting

Less common:

•      Allergic reactions including rash, urticaria, or rarely anaphylaxis

•      Pseudomembranous colitis with prolonged use

•      Elevated liver enzymes

•      Eosinophilia or thrombocytosis

NOT RECOMMENDED IN

•      Patients with known hypersensitivity to ceftazidime, other cephalosporins, or penicillins with severe prior reaction

•      Caution in patients with a history of gastrointestinal disease, especially colitis

•      Renal impairment requires dose adjustment

DRUG INTERACTIONS

Aminoglycoside antibiotics may have additive nephrotoxicity when used concurrently. Probenecid reduces renal clearance of ceftazidime. Loop diuretics such as furosemide may increase nephrotoxicity risk. Chloramphenicol may antagonize the bactericidal effect of ceftazidime and concurrent use should be avoided.

STORAGE INSTRUCTIONS

Store the unreconstituted vial below 25 degrees Celsius. Protect from light. After reconstitution, use within 24 hours if stored at room temperature or 7 days if refrigerated. Keep out of reach of children.
