INDICATION

Fonaz 150mg Cap is a triazole antifungal used as a single-dose treatment for uncomplicated vaginal candidiasis (vaginal yeast infection) in adult women. It may also be used for oral candidiasis and other Candida infections. The 150mg single-dose oral formulation provides effective and convenient antifungal treatment without the need for topical application.

MECHANISM OF ACTION

Fluconazole selectively inhibits the fungal 14-alpha demethylase enzyme in the ergosterol biosynthesis pathway. Inhibition of this enzyme depletes ergosterol in the fungal cell membrane, leading to membrane disruption, altered permeability, and inhibition of fungal cell growth and division. Fluconazole achieves high vaginal tissue concentrations after oral dosing, providing sustained antifungal activity at the site of infection.

DOSAGE AND ADMINISTRATION

•      Uncomplicated vaginal candidiasis: 150mg as a single oral dose

•      Recurrent vaginal candidiasis: consult a physician — may require repeated courses

•      Take with or without food

•      Vaginal symptoms should improve within 1 to 3 days; complete resolution in about 7 days

SIDE EFFECTS

Common:

•      Headache, nausea

•      Abdominal discomfort

•      Diarrhoea

•      Mild rash

Less common:

•      Hepatotoxicity (rare at single dose)

•      QT prolongation (rare)

•      Hypersensitivity including angioedema

NOT RECOMMENDED IN

•      Hypersensitivity to fluconazole or other azoles

•      Concurrent terfenadine, pimozide, or cisapride

•      Significant hepatic impairment

DRUG INTERACTIONS

Even a single 150mg dose can transiently increase plasma levels of warfarin, phenytoin, and sulphonylureas via CYP2C9 and CYP3A4 inhibition. Patients on these drugs should be advised accordingly.
