INDICATION

Folic Acid is indicated for the prevention and treatment of folic acid deficiency, megaloblastic anemia due to folate deficiency, and supplementation during pregnancy to reduce the risk of neural tube defects in the developing fetus. It is also used as adjunctive therapy in patients receiving methotrexate or other antifolate medications.

MECHANISM OF ACTION

Folic acid (Vitamin B9) is a water-soluble B vitamin essential for DNA synthesis, repair, and methylation. After oral ingestion, folic acid is reduced by dihydrofolate reductase to tetrahydrofolate, the metabolically active coenzyme form. Tetrahydrofolate participates in one-carbon transfer reactions required for purine and pyrimidine nucleotide synthesis, amino acid metabolism, and red blood cell maturation. During pregnancy, adequate folate is critical for rapid cell division and proper neural tube closure in the first trimester.

DOSAGE AND ADMINISTRATION

      Adults: 5mg daily for treatment of deficiency, or as directed by a physician

      Pregnancy supplementation: 400mcg to 5mg daily starting at least one month before conception and continuing through the first trimester

      Children: dose as prescribed by a physician based on age and weight

      Swallow with water; may be taken with or without food

      Continue for the duration prescribed by the physician

SIDE EFFECTS

Common:

      Generally, well tolerated at recommended doses

      Mild gastrointestinal discomfort including nausea or bloating in rare cases

Less common:

      Allergic reactions such as skin rash or urticaria in very rare cases

      Large doses may mask Vitamin B12 deficiency by correcting anemia while neurological damage progresses

NOT RECOMMENDED IN

      Patients with known hypersensitivity to folic acid

      Should not be used as sole therapy in pernicious anemia (Vitamin B12 deficiency must be excluded first)

      Caution in patients with undiagnosed anemia

DRUG INTERACTIONS

Phenytoin, carbamazepine, and phenobarbital may reduce folic acid levels and folic acid may reduce the efficacy of these anticonvulsants. Methotrexate and trimethoprim inhibit dihydrofolate reductase, antagonizing folic acid activity. Sulfasalazine impairs folate absorption. Cholestyramine may reduce folic acid absorption.

STORAGE INSTRUCTIONS

Store below 25 degrees Celsius in a cool, dry place. Protect from light and moisture. Keep in original packaging. Keep out of reach of children.
