INDICATION

Fludec Inj is a long-acting injectable antipsychotic (depot formulation) used for the maintenance treatment of schizophrenia and other chronic psychotic disorders in patients who are non-compliant with oral antipsychotic therapy or who require the reliability of a depot injection. A single injection provides sustained antipsychotic plasma levels for 2 to 4 weeks, significantly reducing relapse rates.

MECHANISM OF ACTION

Fluphenazine is a high-potency phenothiazine antipsychotic that primarily blocks dopamine D2 receptors in the mesolimbic and mesocortical pathways, reducing the positive symptoms of psychosis including hallucinations, delusions, and thought disorder. The decanoate ester form releases fluphenazine slowly from the intramuscular injection site over 2 to 4 weeks. It also blocks alpha-1 adrenergic, muscarinic, and H1 histamine receptors, contributing to its sedative and autonomic side effects.

DOSAGE AND ADMINISTRATION

•      Typical dose: 12.5mg to 25mg IM every 2 to 4 weeks

•      Start with a test dose of 12.5mg to assess tolerability

•      Adjust dose and interval based on clinical response

•      Administer by deep intramuscular injection into the gluteal muscle using a dry syringe

•      Do not administer intravenously

SIDE EFFECTS

Common:

•      Extrapyramidal symptoms (EPS): tremor, rigidity, akathisia, dystonia

•      Sedation

•      Anticholinergic effects: dry mouth, constipation, urinary retention

•      Weight gain

Less common:

•      Tardive dyskinesia with long-term use (potentially irreversible)

•      Neuroleptic malignant syndrome (rare but life-threatening)

•      QT prolongation and ventricular arrhythmias

•      Hyperprolactinemia causing galactorrhea and sexual dysfunction

NOT RECOMMENDED IN

•      Comatose states or CNS depression from any cause

•      Blood dyscrasias

DRUG INTERACTIONS

CNS depressants (alcohol, sedatives, opioids) have additive depressant effects. Anticholinergic drugs increase the risk of anticholinergic toxicity. Concurrent use with lithium increases the risk of extrapyramidal side effects. QT-prolonging agents increase arrhythmia risk. Antihypertensives may cause additive hypotension. Antacids reduce the absorption of oral fluphenazine formulations.
