INDICATION

Flucate Injection is a long-acting depot antipsychotic indicated for the maintenance treatment of chronic schizophrenia and other psychotic disorders in patients who are non-compliant with oral antipsychotic therapy or who benefit from prolonged drug release following a single injection.

MECHANISM OF ACTION

Fluphenazine decanoate is the long-chain ester of fluphenazine, a high-potency typical antipsychotic of the phenothiazine class. Following intramuscular injection, the decanoate ester is slowly hydrolyzed in the muscle depot, releasing free fluphenazine over 2 to 4 weeks. Fluphenazine blocks postsynaptic dopamine D2 receptors in the mesolimbic pathway, reducing positive symptoms of psychosis including hallucinations, delusions, and thought disorder. It also blocks dopamine receptors in the nigrostriatal pathway, which accounts for its extrapyramidal side effects.

DOSAGE AND ADMINISTRATION

•      Administered by deep intramuscular injection only, typically into the gluteal muscle

•      Initial test dose: 12.5mg (0.5ml) for antipsychotic-naive patients to assess tolerability

•      Maintenance dose: 12.5mg to 100mg every 2 to 4 weeks depending on response and tolerability

•      Dose and interval are individualized under psychiatric supervision

•      Never administer intravenously

•      Rotate injection sites to reduce local tissue reactions

SIDE EFFECTS

Common:

•      Extrapyramidal side effects including tremor, rigidity, and akathisia

•      Sedation or drowsiness

•      Dry mouth, blurred vision, or constipation (anticholinergic effects)

Less common:

•      Tardive dyskinesia with long-term use

•      Neuroleptic malignant syndrome in rare cases

•      Postural hypotension

•      Galactorrhea or menstrual irregularities due to prolactin elevation

NOT RECOMMENDED IN

•      Patients in comatose states or with CNS depression from alcohol, barbiturates, or opioids

•      Patients with known hypersensitivity to phenothiazines

•      Parkinson's disease or pre-existing movement disorders

•      History of neuroleptic malignant syndrome

DRUG INTERACTIONS

CNS depressants including alcohol, benzodiazepines, and opioids have additive sedative effects. Anticholinergic drugs may worsen constipation and urinary retention. Lithium combined with antipsychotics may increase neurotoxicity risk. Fluphenazine may reduce the anticonvulsant effect of antiepileptic drugs, requiring dose adjustment.
