INDICATION
Flow RL-D 1000ML is a sterile combined Ringer's Lactate and dextrose intravenous solution indicated for fluid and electrolyte replacement with concurrent caloric support. It is used in post-operative fluid management, dehydration with electrolyte loss, surgical maintenance fluid therapy, and in burn patients requiring balanced fluid with glucose.
MECHANISM OF ACTION
Flow RL-D combines the balanced electrolyte profile of Ringer's Lactate with the energy provision of dextrose 5%. Ringer's Lactate provides sodium, potassium, calcium, and lactate in physiological proportions. Lactate is metabolized by the liver to bicarbonate, providing a mild buffering effect. The addition of dextrose 5% provides 170 kcal/L of energy from glucose metabolism and contributes free water after oxidation. This combination is superior to plain dextrose saline in surgical and trauma patients where acid-base balance and electrolyte composition are critical.
DOSAGE AND ADMINISTRATION
• Administered intravenously under medical supervision
• Dosing is individualized based on surgical requirements, fluid losses, and metabolic status
• Standard intraoperative maintenance: 3 to 8 ml/kg/hour adjusted to surgical bleeding and insensible losses
• Monitor blood glucose, lactate levels, and serum electrolytes during administration
• Calcium in the solution is incompatible with certain medications; verify compatibility before co-infusion
SIDE EFFECTS
Common:
• Hyperglycemia in diabetic patients or with rapid infusion
• Mild fluid retention with excessive volumes
Less common:
• Hyperlactatemia in patients with severe hepatic or mitochondrial dysfunction
• Electrolyte imbalances with prolonged use without monitoring
NOT RECOMMENDED IN
• Patients with lactic acidosis or severe hepatic failure
• Do not mix with blood products due to calcium content
• Uncontrolled hyperglycemia without insulin management
DRUG INTERACTIONS
Calcium in the solution precipitates with phosphate-containing IV medications and certain cephalosporins. Incompatible with amphotericin B. Monitor insulin requirements with glucose component.
