INDICATION

Flow DS 1/2 500ML is a sterile combined dextrose and hypotonic saline infusion indicated for maintenance fluid therapy, post-operative fluid management in adults, rehydration in mild to moderate hypotonic dehydration, and as a maintenance solution when a combination of free water, electrolytes, and caloric support is required.

MECHANISM OF ACTION

DS 1/2 combines the benefits of dextrose 5% and half-normal saline (0.45% NaCl). It provides 170 kcal/L from glucose, 77 mEq/L of sodium, and 77 mEq/L of chloride, making it suitable for maintenance rather than resuscitation. The hypotonic saline component provides electrolyte replacement without the full sodium load of normal saline. Glucose metabolism generates free water, contributing to intracellular hydration. This formulation is widely used for daily maintenance fluid requirements.

DOSAGE AND ADMINISTRATION

•      Administered intravenously under physician supervision

•      Maintenance dose for adults: 1.5 to 2.5 ml/kg/hour or as calculated based on body weight and clinical needs

•      Pediatric dosing follows the Holliday-Segar formula under medical supervision

•      Monitor serum sodium, potassium, and blood glucose regularly

•      Adjust rate based on fluid balance, urine output, and clinical parameters

•      Do not use for acute resuscitation or hypovolemia requiring isotonic fluids

SIDE EFFECTS

Common:

•      Mild electrolyte shifts with prolonged use

•      Hyperglycemia in diabetic or critically ill patients

Less common:

•      Hyponatremia with excessive administration, particularly in pediatric patients

•      Fluid overload in patients with cardiac or renal compromise

NOT RECOMMENDED IN

•      Acute circulatory failure requiring isotonic resuscitation
