INDICATION

Flow Dex 5% 1000ML is a sterile isotonic dextrose solution for intravenous use. It is indicated for hydration, provision of caloric support, correction of mild hypoglycemia, dilution of compatible intravenous medications, and as a vehicle for drug administration when sodium restriction is necessary.

MECHANISM OF ACTION

Dextrose 5% in water provides approximately 170 kcal per liter and is initially iso-osmolar with plasma (252 mOsm/L). Following infusion, glucose is rapidly metabolized, leaving free water that distributes throughout all body fluid compartments. Unlike normal saline, D5W does not contribute to sodium load, making it suitable in hypernatremia or when sodium-free fluid replacement is required. Dextrose is oxidized to CO2 and water through glycolysis and the citric acid cycle.

DOSAGE AND ADMINISTRATION

•      Administered intravenously under physician supervision

•      Standard adult maintenance rate: 125 ml per hour or as individually calculated

•      For drug dilution: follow specific medication dilution instructions

•      Monitor blood glucose regularly, especially in diabetic patients or during prolonged infusion

•      Monitor serum electrolytes and fluid balance throughout therapy

•      Adjust rate based on clinical response and laboratory results

SIDE EFFECTS

Common:

•      Hyperglycemia in diabetic or critically ill patients

•      Fluid overload with excessive or rapid infusion

 

Less common:

•      Hyponatremia with large volumes due to free water distribution

•      Phlebitis at infusion site with prolonged peripheral administration

NOT RECOMMENDED IN

•      Uncontrolled diabetes without glycemic monitoring

•      Use during neurosurgery or head injury due to risk of worsening cerebral edema

•      Hypersensitivity to dextrose

DRUG INTERACTIONS

Blood and blood products must not be mixed directly with dextrose solutions as red cell aggregation may occur. Amphotericin B is incompatible with dextrose. Phenytoin precipitates in dextrose solutions. Monitor glycemia with concurrent corticosteroid use.

STORAGE INSTRUCTIONS

Store at room temperature between 15 to 25 degrees Celsius. Protect from freezing and direct sunlight. Do not use if solution is discolored or the container is damaged. Single use only.
