INDICATION

Floaid 10mg Tab is a leukotriene receptor antagonist used for the prophylaxis and long-term treatment of asthma in adults and adolescents aged 15 years and above, and for the relief of symptoms of seasonal and perennial allergic rhinitis. It reduces the frequency of asthma attacks, improves lung function, and controls rhinitis symptoms without the sedating side effects of antihistamines.

MECHANISM OF ACTION

Montelukast selectively blocks cysteinyl leukotriene receptors (CysLT1) on airway smooth muscle, eosinophils, and mast cells. Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are potent inflammatory mediators released during allergic reactions that cause bronchoconstriction, increased mucus secretion, airway oedema, and eosinophil recruitment. By blocking CysLT1 receptors, montelukast reduces airway inflammation and bronchospasm, improves airflow, and decreases the late-phase response after allergen exposure.

DOSAGE AND ADMINISTRATION

•      Asthma and allergic rhinitis (adults and adolescents above 15 years): 10mg once daily in the evening

•      The evening dosing timing is recommended due to the nocturnal nature of asthma symptoms

•      Can be taken with or without food

•      Not for use as a reliever inhaler — continue short-acting bronchodilator for acute attacks

•      Effects may be apparent within 2 to 4 weeks of regular use

SIDE EFFECTS

Common:

•      Headache

•      Abdominal pain, dyspepsia

•      Elevated liver enzymes (rare)

•      Fatigue

Less common:

•      Neuropsychiatric events including agitation, depression, suicidal ideation (rare but reported)

•      Hypersensitivity reactions including angioedema and skin rash

•      Eosinophilic granulomatosis with polyangiitis (Churg-Strauss) — rare, usually on steroid tapering

NOT RECOMMENDED IN

•      Known hypersensitivity to montelukast

•      Not for use in acute asthma attacks

•      Use with caution in patients with a history of psychiatric disorders

DRUG INTERACTIONS

Rifampicin and other CYP3A4 inducers may reduce montelukast plasma levels. Phenobarbital may reduce montelukast efficacy through enzyme induction. No clinically relevant interactions with most commonly used asthma medications. Montelukast does not inhibit the cytochrome P450 system at therapeutic doses.
