INDICATION

Flagyl Plus Suspension is indicated for the treatment of intestinal amoebiasis, amoebic liver abscess, and mixed intestinal infections caused by susceptible protozoa including Entamoeba histolytica, Trichomonas vaginalis, and Giardia lamblia in pediatric and adult patients.

MECHANISM OF ACTION

Metronidazole is a nitroimidazole antiprotozoal and antibacterial agent. It is reduced intracellularly by anaerobic organisms, generating cytotoxic free radicals that damage DNA strands and inhibit nucleic acid synthesis, leading to cell death in anaerobic bacteria and protozoa. Diloxanide furoate is a luminal amebicide that acts locally in the intestinal lumen. It is hydrolyzed to diloxanide, which directly kills trophozoites and cysts of Entamoeba histolytica in the colon. The combination eradicates both tissue-invasive amoeba through metronidazole and luminal cysts through diloxanide, providing complete treatment.

DOSAGE AND ADMINISTRATION

•      Children: 7.5mg/kg of metronidazole component three times daily for 5 to 10 days

•      Standard pediatric dose: 5ml three times daily for a child of average weight

•      Adults: 10ml three times daily for 5 to 10 days or as directed by the physician

•      Shake the bottle well before each use

•      May be taken with food to reduce gastric irritation

•      Complete the full prescribed course even if symptoms improve early

SIDE EFFECTS

Common:

•      Nausea or metallic taste in the mouth

•      Loss of appetite

•      Mild stomach cramps or diarrhea

Less common:

•      Headache or dizziness

•      Dark or discolored urine due to metronidazole metabolites

•      Peripheral neuropathy with prolonged high-dose use

NOT RECOMMENDED IN

•      First trimester of pregnancy due to potential teratogenicity of metronidazole

•      Patients with known hypersensitivity to metronidazole, diloxanide, or nitroimidazole derivatives

•      Patients with severe hepatic impairment

•      Avoid alcohol consumption during and for 48 hours after therapy due to disulfiram-like reaction

DRUG INTERACTIONS

Metronidazole significantly enhances the anticoagulant effect of warfarin by inhibiting its metabolism, requiring close INR monitoring. It inhibits CYP2C9 and may increase plasma levels of phenytoin. Concurrent alcohol use causes a severe disulfiram-like reaction with flushing, vomiting, and tachycardia. Cimetidine inhibits metronidazole metabolism and may increase its plasma levels.
