INDICATION

First GTN Ointment is indicated for the treatment and prevention of angina pectoris. It provides relief during acute anginal attacks and is used as prophylactic therapy for patients who experience frequent episodes of chest pain caused by coronary artery disease. It is also used in the management of chronic anal fissures to reduce internal anal sphincter tone.

MECHANISM OF ACTION

Glyceryl trinitrate is an organic nitrate that releases nitric oxide (NO) following enzymatic denitration. Nitric oxide activates soluble guanylate cyclase, increasing cyclic GMP levels in vascular smooth muscle cells. This causes dephosphorylation of myosin light chains, leading to smooth muscle relaxation and vasodilation. In cardiac use, venous dilation reduces preload, reducing myocardial oxygen demand. In anal fissure management, the ointment relaxes the internal anal sphincter, improving blood flow and allowing tissue healing.

DOSAGE AND ADMINISTRATION

•      For angina: apply a thin ribbon of ointment (1 to 2 inches) to a measured dose paper and spread on clean, hairless skin such as the chest or upper arm

•      For anal fissure: apply a small amount inside and around the anal canal every 12 hours as directed by a physician

•      Wash hands thoroughly after application to avoid accidental absorption through the palms

•      Rotate application sites to prevent skin irritation and tolerance

•      Do not apply to broken or irritated skin

•      Use for the prescribed duration only

SIDE EFFECTS

Common:

•      Headache, often throbbing, particularly at the start of therapy

•      Flushing of the face or neck

•      Dizziness or lightheadedness on standing

Less common:

•      Postural hypotension or fainting

•      Tachycardia or palpitations

•      Skin irritation or contact dermatitis at application site

•      Tolerance with continuous use

NOT RECOMMENDED IN

•      Patients taking phosphodiesterase-5 inhibitors such as sildenafil, tadalafil, or vardenafil due to severe hypotension risk

•      Severe hypotension or hypovolemia

•      Hypertrophic obstructive cardiomyopathy

•      Raised intracranial pressure

•      Hypersensitivity to nitrates or nitrites

DRUG INTERACTIONS

Concurrent use with phosphodiesterase-5 inhibitors (sildenafil, tadalafil) is absolutely contraindicated due to risk of fatal hypotension. Antihypertensives and other vasodilators may have additive blood pressure lowering effects. Alcohol enhances the hypotensive effect. Ergotamine may have reduced efficacy due to vasoconstrictive antagonism.
