INDICATION
Filgen Injection is indicated for the reduction of the duration of neutropenia and its associated complications in patients receiving myelosuppressive chemotherapy for malignancies. It is also indicated for mobilization of hematopoietic progenitor cells for autologous peripheral blood stem cell collection, treatment of severe chronic neutropenia, and management of neutropenia in HIV-infected patients.
MECHANISM OF ACTION
Filgrastim is a recombinant human granulocyte colony-stimulating factor (rhG-CSF) that binds to specific cell surface receptors on myeloid progenitor cells and mature neutrophils. This binding activates intracellular JAK/STAT signaling pathways, stimulating proliferation, differentiation, and functional activation of neutrophil precursors. Filgrastim significantly reduces the nadir and duration of neutropenia following chemotherapy, shortening the window of infection risk. It also mobilizes hematopoietic stem cells from bone marrow into peripheral blood for harvesting.
DOSAGE AND ADMINISTRATION
· For chemotherapy-induced neutropenia: 5 mcg/kg/day by subcutaneous injection or short IV infusion, starting 24 hours after the last dose of chemotherapy and continuing until the neutrophil count has recovered to the normal range.
· For stem cell mobilization: 10 mcg/kg/day subcutaneously for 4 to 5 days.
· Do not administer within 24 hours before or after cytotoxic chemotherapy.
· Doses are adjusted based on absolute neutrophil count (ANC) monitoring.
SIDE EFFECTS
Common:
• Bone pain, particularly in the spine, pelvis, and sternum
• Mild nausea or fatigue
• Injection site redness or swelling
Less common:
• Splenomegaly with prolonged use, rarely splenic rupture
• Pulmonary adverse events including acute respiratory distress syndrome
• Sickle cell crisis in patients with sickle cell disease
• Elevated serum uric acid and lactate dehydrogenase
NOT RECOMMENDED IN
• Hypersensitivity to filgrastim or E. coli-derived proteins
• Patients with sickle cell disease due to risk of crisis
• Myeloid malignancies without specialist guidance
• Concurrent myelosuppressive radiotherapy
DRUG INTERACTIONS
• Cytotoxic chemotherapy agents should not be given within 24 hours of filgrastim administration
• Lithium may potentiate neutrophil production, requiring dose adjustment monitoring
• Topoisomerase II inhibitors such as etoposide may have enhanced sensitivity in high-risk scenarios
STORAGE INSTRUCTIONS
Store in a refrigerator between 2 to 8 degrees Celsius. Do not freeze. Protect from light. Allow to reach room temperature before administration. Once removed from refrigerator, use within 24 hours. Keep out of reach of children.
