INDICATION

Ferium 500mg Injection is indicated for the treatment of iron deficiency anemia in adults when oral iron therapy is ineffective, not tolerated, or when rapid iron repletion is clinically required. It is used in patients with chronic kidney disease, inflammatory bowel disease, post-surgical iron deficiency, and iron deficiency during pregnancy.

MECHANISM OF ACTION

Ferric carboxymaltose is a non-dextran, non-reducible iron complex that releases iron in a controlled manner to target sites including the bone marrow and reticuloendothelial system. Following intravenous administration, the complex is taken up by macrophages, where iron is liberated and incorporated into ferritin and hemosiderin. This iron is then made available to erythroid precursor cells for hemoglobin synthesis. The carboxymaltose shell provides stability, prevents free iron toxicity, and allows administration of large iron doses in a single infusion.

DOSAGE AND ADMINISTRATION

·        Administered as a slow intravenous injection or infusion under medical supervision.

·        The dose is calculated based on body weight and hemoglobin deficit using a standard formula.

·        Single doses up to 1000mg may be given over 15 minutes as an IV infusion or as a slow IV push.

·        Ferium 500mg may be given undiluted as a slow IV injection at a rate not exceeding 100mg per minute.

·        Never administer by intramuscular or subcutaneous injection.

SIDE EFFECTS

Common:

•      Nausea or headache

•      Dizziness or flushing during or after infusion

•      Mild injection site reaction or discoloration

Less common:

•      Transient hypophosphatemia with repeated high doses

•      Allergic or hypersensitivity reactions including anaphylaxis in rare cases

•      Hypertension or hypotension during infusion

NOT RECOMMENDED IN

•      Iron overload states including hemochromatosis or hemosiderosis

•      Anemia not caused by iron deficiency

•      Hypersensitivity to ferric carboxymaltose or other parenteral iron preparations

•      First trimester of pregnancy

DRUG INTERACTIONS

•      Oral iron absorption is reduced for up to 5 days after parenteral iron administration

•      Avoid concurrent use with other parenteral iron formulations

•      ACE inhibitors may increase risk of systemic reactions during infusion
