INDICATION

FDL RL 1000ml (Ringer's Lactate) is a sterile, balanced isotonic crystalloid solution for intravenous infusion. It is indicated for the resuscitation and replacement of fluid losses from burns, trauma, surgery, and severe diarrhea. It is also used for perioperative fluid maintenance and as a vehicle for compatible intravenous medications.

MECHANISM OF ACTION

Ringer's Lactate contains sodium chloride, potassium chloride, calcium chloride, and sodium lactate in water for injection. Its composition closely resembles the electrolyte concentration of plasma, making it a physiologically balanced solution. Lactate is metabolized by the liver to bicarbonate, providing a mild buffering effect that prevents hyperchloremic acidosis, which can occur with large-volume normal saline infusions. The balanced electrolyte profile makes it particularly useful in conditions requiring large-volume resuscitation.

DOSAGE AND ADMINISTRATION

Administered intravenously under medical supervision. The volume and rate of infusion are individualized based on the patient's clinical condition, degree of dehydration or blood loss, age, and body weight. For general fluid resuscitation, initial rates of 500 ml to 1000 ml over 30 to 60 minutes may be used in acute settings, with subsequent rates adjusted based on clinical response, urine output, and vital signs.

SIDE EFFECTS

Common:

      Infusion site discomfort or mild phlebitis

      Mild fluid retention with high infusion rates

Less common:

      Electrolyte imbalances with large volumes, including hypokalemia or hypercalcemia

      Metabolic alkalosis with excessive lactate metabolism in patients with good hepatic function

      Pulmonary or peripheral edema in patients with cardiac or renal compromise

NOT RECOMMENDED IN

      Patients with lactic acidosis or severe hepatic impairment who cannot metabolize lactate

      Hyperkalemia or severe hypercalcemia

      Congestive heart failure without careful monitoring

      Not suitable for use with blood products due to calcium content causing clot formation

DRUG INTERACTIONS

      Calcium in the solution may precipitate with phosphate-containing medications if mixed directly

      Incompatible with amphotericin B and some cephalosporins when mixed

      Concurrent use with potassium-sparing diuretics may lead to hyperkalemia

STORAGE INSTRUCTIONS

Store at room temperature between 15 to 25 degrees Celsius. Protect from freezing and direct sunlight. Do not use if the solution is discolored or cloudy, or if the container is damaged. Use immediately after opening.0
