INDICATION

Exempa-Lin Met XR 25mg/5mg/1000mg is indicated as an adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus not adequately controlled on dual antidiabetic therapy, requiring a higher empagliflozin dose.

MECHANISM OF ACTION

This higher-strength formulation contains 25mg empagliflozin in combination with linagliptin 5mg and metformin 1000mg. Empagliflozin at 25mg achieves greater urinary glucose excretion by more complete SGLT2 receptor inhibition. Linagliptin enhances incretin-mediated insulin secretion. Metformin suppresses hepatic gluconeogenesis. DOSAGE AND ADMINISTRATION

·        One tablet orally once or twice daily with a meal.

·        Swallow whole and do not split, crush, or chew.

·        Dose adjustments are made based on renal function.

·        The 25mg empagliflozin dose is appropriate for patients who require intensified glycemic control.

·        Monitor blood glucose regularly and adjust as needed.

SIDE EFFECTS

Common:

•      Nausea or diarrhea, common at treatment initiation with metformin

•      Urogenital infections including genital mycotic infections

•      Increased urinary frequency due to SGLT2 inhibitor

Less common:

•      Volume depletion and dehydration particularly in elderly patients

•      Hypoglycemia when combined with insulin or sulfonylurea

•      Rare cases of diabetic ketoacidosis

•      Patients with eGFR below 30 ml/min/1.73m2

•      Concurrent use in type 1 diabetes

•      Severe hepatic or renal impairment

•      History of ketoacidosis

•      Hypersensitivity to any component

DRUG INTERACTIONS

•      Diuretics increase risk of dehydration and hypotension with empagliflozin

•      Alcohol may potentiate lactic acidosis risk with metformin

•      Iodinated contrast agents require temporary discontinuation of metformin
