INDICATION

Exempa-Lin Met XR 10mg/5mg/1000mg is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with three antidiabetic agents in a single tablet is appropriate.

MECHANISM OF ACTION

This triple-combination tablet works through three complementary mechanisms. Empagliflozin inhibits sodium-glucose cotransporter 2 (SGLT2) in the renal proximal tubule, reducing glucose reabsorption and promoting urinary glucose excretion. Linagliptin is a DPP-4 inhibitor that prolongs the action of incretin hormones GLP-1 and GIP, enhancing glucose-dependent insulin secretion and reducing glucagon levels. Metformin reduces hepatic glucose production and improves peripheral insulin sensitivity, primarily through AMPK activation.

DOSAGE AND ADMINISTRATION

·        One tablet orally once or twice daily with meals.

·        Swallow whole and do not crush, chew, or split.

·        Dose should be individualized based on the patient's current antidiabetic regimen, renal function, and glycemic response.

·        Follow the prescribing physician's instructions. Monitor renal function regularly.

SIDE EFFECTS

Common:

•      Nausea, vomiting, or diarrhea, particularly at initiation

•      Genital mycotic infections due to SGLT2 inhibition

•      Urinary tract infections

Less common:

•      Hypoglycemia, particularly if combined with insulin or sulfonylureas

•      Diabetic ketoacidosis in rare cases

DRUG INTERACTIONS

•      Insulin or insulin secretagogues increase hypoglycemia risk

•      Loop diuretics increase risk of volume depletion with SGLT2 inhibitor

•      Cimetidine may increase metformin plasma levels
