INDICATION

Epokine 4000IU Injection is indicated for the treatment of anemia associated with chronic renal failure, anemia in cancer patients receiving chemotherapy, and anemia in HIV-infected patients treated with zidovudine. It is also used to reduce the need for allogeneic blood transfusions in surgical patients.

MECHANISM OF ACTION

Erythropoietin is a glycoprotein hormone that binds to specific receptors on erythroid progenitor cells in the bone marrow. This binding stimulates red blood cell (erythrocyte) production and differentiation by activating intracellular JAK2/STAT5 signaling pathways. The result is increased hemoglobin levels and improved oxygen-carrying capacity of the blood.

DOSAGE AND ADMINISTRATION

·        Administered subcutaneously or intravenously under physician supervision. For chronic renal failure anemia: 50 to 150 IU/kg three times per week, with dose adjustments based on hemoglobin response.

·        For chemotherapy-related anemia: 150 IU/kg three times per week or 40,000 IU once weekly.

·        Target hemoglobin should not exceed 12 g/dl. Titrate dose individually based on response.

SIDE EFFECTS

Common:

•      Injection site pain or redness

•      Headache

•      Flu-like symptoms including fever and fatigue

Less common:

•      Hypertension requiring management

•      Thromboembolic events including deep vein thrombosis

•      Pure red cell aplasia with long-term use in rare cases

NOT RECOMMENDED IN

•      Patients with uncontrolled hypertension

•      Patients with pure red cell aplasia related to prior erythropoietin use

•      Patients with hemoglobin at or above 12 g/dl

•      Patients with hypersensitivity to erythropoietin or any excipient

DRUG INTERACTIONS

•      Angiotensin-converting enzyme inhibitors may reduce effectiveness

•      Concurrent iron deficiency must be corrected for optimal response

•      Hydroxyurea may interfere with erythropoiesis

STORAGE INSTRUCTIONS
