INDICATION

Ephagent E/E Drop 0.3% is indicated for the treatment of external bacterial infections of the eye and ear caused by gentamicin-susceptible organisms. Eye indications include bacterial conjunctivitis, blepharitis, keratitis, and corneal ulcers. Ear indications include otitis externa and acute bacterial otitis media with perforated eardrum.

MECHANISM OF ACTION

Gentamicin is an aminoglycoside antibiotic that exerts bactericidal activity by irreversibly binding to the 30S ribosomal subunit of susceptible bacteria. This causes misreading of the mRNA template, leading to faulty protein synthesis and ultimately cell death. It is active against a broad range of gram-negative organisms including Pseudomonas aeruginosa, Staphylococcus aureus, and Streptococcus species.

DOSAGE AND ADMINISTRATION

·        For eyes: instill 1 to 2 drops into the affected eye every 4 hours.

·        In severe infections, up to 2 drops every hour may be used and frequency reduced as improvement occurs.

·        For ears: instill 3 to 4 drops into the affected ear three to four times daily. Continue for the full prescribed duration. Do not touch the dropper tip to any surface.

SIDE EFFECTS

Common:

•      Mild stinging or burning on instillation

•      Temporary blurring of vision after eye use

Less common:

•      Allergic conjunctivitis or skin hypersensitivity

•      Local irritation of the eyelid or ear canal

•      Overgrowth of resistant organisms with prolonged use

NOT RECOMMENDED IN

•      Patients with hypersensitivity to gentamicin or other aminoglycosides

•      Viral or fungal ocular or ear infections

•      Do not use ear drops if eardrum integrity is uncertain without physician guidance

DRUG INTERACTIONS

•      Concurrent systemic aminoglycosides may increase risk of ototoxicity or nephrotoxicity

•      Other ototoxic medications such as loop diuretics should be used with caution simultaneously

Store below 25 degrees Celsius. Protect from light. Discard 28 days after first opening. Keep out of reach of children.
