INDICATION

Empaa-M 12.5/500mg Tab is a fixed-dose combination for type 2 diabetes management in adults who require both SGLT2 inhibitor and metformin therapy. It is used as an adjunct to diet and exercise when the individual components do not provide adequate glycemic control. It also confers cardiovascular and renal protective benefits in high-risk diabetic patients.

MECHANISM OF ACTION

Same as Elzanor 12.5/500mg. Empagliflozin blocks SGLT2-mediated renal glucose reabsorption, causing glucosuria and lowering blood glucose. Metformin reduces hepatic glucose output and improves insulin sensitivity. The dual mechanism provides complementary glycemic control without significant hypoglycemia risk.

DOSAGE AND ADMINISTRATION

•      1 tablet twice daily with meals

•      Adjust based on patient's current antidiabetic regimen

•      Monitor eGFR regularly

•      Hold before procedures requiring iodinated contrast

•      Not for use when eGFR is below 45ml/min/1.73m2

SIDE EFFECTS

Common:

•      Genital mycotic infections

•      Urinary tract infections

•      Nausea, diarrhoea from metformin

•      Increased urination

Less common:

•      Diabetic ketoacidosis

•      Hypotension, dehydration

•      Acute kidney injury

•      Lactic acidosis (rare)

NOT RECOMMENDED IN

•      eGFR below 45ml/min/1.73m2

•      Type 1 diabetes or DKA

•      Severe hepatic impairment

•      Pregnancy and breastfeeding

DRUG INTERACTIONS

Same as Elzanor. Insulin and sulphonylureas increase hypoglycemia risk. Diuretics increase dehydration risk. Alcohol increases lactic acidosis risk.
