INDICATION
Elzanor 12.5/500mg Tab is a fixed-dose combination of an SGLT2 inhibitor and a biguanide used as an adjunct to diet and exercise in adults with type 2 diabetes mellitus who are not adequately controlled on empagliflozin or metformin alone. The combination also offers cardiovascular and renal protective benefits in diabetic patients at high risk.
MECHANISM OF ACTION
Empagliflozin selectively inhibits sodium-glucose cotransporter-2 (SGLT2) in the proximal renal tubule, promoting urinary glucose excretion independent of insulin, lowering blood glucose, blood pressure, and body weight. Metformin reduces hepatic glucose production via AMPK activation, improves peripheral insulin sensitivity, and decreases intestinal glucose absorption. Together, these complementary mechanisms provide superior glycemic control with lower risk of hypoglycemia than sulphonylureas combinations.
DOSAGE AND ADMINISTRATION
• 1 tablet twice daily with meals
• Dose is individualized based on current empagliflozin and metformin doses
• Swallow tablets whole with water
• Monitor renal function before and during therapy
• Not recommended when eGFR is below 45ml/min/1.73m2
SIDE EFFECTS
Common:
• Genital mycotic infections (yeast infections)
• Urinary tract infections
• Nausea, diarrhoea from metformin component
• Polyuria, increased urination
Less common:
• Diabetic ketoacidosis (even with near-normal blood glucose)
• Volume depletion and hypotension
• Acute kidney injury with dehydration
• Lactic acidosis from metformin (rare, mainly in renal failure)
NOT RECOMMENDED IN
• eGFR below 45ml/min/1.73m2
• Diabetic ketoacidosis
• Type 1 diabetes
• Severe hepatic impairment
• Iodinated contrast media procedures (hold metformin 48 hours before)
DRUG INTERACTIONS
Insulin and sulphonylureas increase hypoglycemia risk when combined. Diuretics and antihypertensives increase volume depletion risk. NSAIDs and ACE inhibitors increase acute kidney injury risk with empagliflozin. Alcohol increases lactic acidosis risk from metformin.
