INDICATION

Egolix 150mg is indicated for the management of moderate to severe pain associated with endometriosis, including dysmenorrhea and non-menstrual pelvic pain.

MECHANISM OF ACTION

Elagolix is an oral, non-peptide, gonadotropin-releasing hormone (GnRH) receptor antagonist. It binds competitively to GnRH receptors in the pituitary gland, blocking endogenous GnRH signaling. This results in dose-dependent suppression of luteinizing hormone (LH) and follicle-stimulating hormone (FSH), leading to reduced estradiol and progesterone levels. By lowering estrogen, elagolix limits the growth and activity of endometrial tissue, thereby reducing associated pain.

DOSAGE AND ADMINISTRATION

The recommended dose is 150mg orally once daily for up to 24 months, or as directed by the physician. In patients with moderate hepatic impairment, use is limited to 6 months. Treatment should begin within 7 days from the onset of menstruation. Pregnancy must be excluded before starting therapy.

SIDE EFFECTS

Common:

•      Hot flushes or hot flashes

Less common:

•      Bone mineral density reduction with prolonged use

•      Mood changes, depression, or anxiety

•      Altered menstrual bleeding patterns

•      Elevations in liver enzymes

DRUG INTERACTIONS

•      Strong CYP3A4 inducers such as rifampin may reduce elagolix blood levels

•      P-glycoprotein inhibitors such as cyclosporine may increase elagolix exposure

•      Elagolix may reduce the effectiveness of hormonal contraceptives
