INDICATION

Ebak 10mg Tab is a second-generation, non-sedating antihistamine used for the relief of symptoms associated with seasonal allergic rhinitis (hay fever), perennial allergic rhinitis, and chronic idiopathic urticaria (hives). It effectively reduces sneezing, nasal discharge, nasal itching, watery eyes, and skin itching and redness without causing significant sedation or impairment.

MECHANISM OF ACTION

Ebastine is a selective and long-acting H1 histamine receptor antagonist. It competitively blocks histamine binding at peripheral H1 receptors on blood vessels, bronchi, and sensory nerve endings, preventing the vasodilation, increased capillary permeability, smooth muscle contraction, and nerve sensitization caused by histamine. Ebastine has minimal penetration of the blood-brain barrier, resulting in a low incidence of sedation. Its active metabolite, carebastine, has a prolonged half-life providing 24-hour allergy relief.

DOSAGE AND ADMINISTRATION

•      Adults and adolescents above 12 years: 10mg once daily

•      Severe symptoms: 20mg once daily may be used

•      Children 6 to 11 years: 5mg once daily (lower strength formulation required)

•      Take with or without food; once-daily dosing provides all-day control

•      No dose adjustment needed in mild to moderate renal or hepatic impairment

SIDE EFFECTS

Common:

•      Headache

•      Dry mouth, somnolence (infrequent)

•      Nausea, abdominal pain

•      Dizziness

Less common:

•      QT prolongation at high doses, particularly when combined with other QT-prolonging drugs

•      Hypersensitivity reactions including angioedema

•      Palpitations

NOT RECOMMENDED IN

•      Known hypersensitivity to Ebastine

•      Concomitant use with strong CYP3A4 inhibitors (ketoconazole, erythromycin) increases QT risk; use with caution

•      Known QT prolongation or hypokalaemia

•      Not recommended in children under 2 years

DRUG INTERACTIONS

Ebastine is metabolized by CYP3A4. Strong CYP3A4 inhibitors such as ketoconazole and erythromycin significantly increase plasma Ebastine levels, raising the risk of QT prolongation. Ebastine should be used with caution alongside other QT-prolonging medications. Grapefruit juice may inhibit CYP3A4 and increase Ebastine exposure.
