INDICATION

Doxium 500mg Cap is a vasoprotective agent used for the treatment of chronic venous insufficiency, diabetic microangiopathy (diabetic retinopathy, nephropathy), and hemorrhoids. It reduces capillary permeability, improves blood viscosity, and strengthens the vascular wall in patients with microvascular complications of diabetes. It is also used in lymphoedema and as an adjunct in the management of chronic venous disease.

MECHANISM OF ACTION

Calcium dobesilate reduces increased capillary permeability by inhibiting the action of platelet-activating factor and prostaglandins on the vascular endothelium. It reduces plasma viscosity by decreasing fibrinogen and improving red blood cell deformability, thereby enhancing microvascular blood flow. It also inhibits the basement membrane thickening seen in diabetic microangiopathy, slowing disease progression. Additionally, it has antioxidant properties and reduces vascular endothelial growth factor (VEGF) expression.

DOSAGE AND ADMINISTRATION

•      Diabetic retinopathy and microangiopathy: 500mg two to three times daily

•      Chronic venous insufficiency: 500mg twice daily

•      Hemorrhoids (acute phase): 500mg three times daily for 4 to 6 weeks

•      Take with meals to reduce gastrointestinal side effects

•      Long-term treatment is usually required for chronic conditions

SIDE EFFECTS

Common:

•      Nausea, abdominal discomfort

•      Diarrhoea

•      Skin rash (rare)

•      Headache

Less common:

•      Agranulocytosis (rare but serious — monitor blood count with prolonged use)

•      Fever and joint pain (rare hypersensitivity reaction)

•      Elevated liver enzymes

NOT RECOMMENDED IN

•      Known hypersensitivity to calcium dobesilate

•      History of agranulocytosis

•      Use with caution in patients with pre-existing blood dyscrasias

DRUG INTERACTIONS
