INDICATION

Dologab 75mg Cap is prescribed for neuropathic pain associated with diabetic peripheral neuropathy, postherpetic neuralgia, and spinal cord injury. It is also used as adjunctive therapy for partial-onset seizures in adults, and for the management of fibromyalgia and generalized anxiety disorder. It provides effective relief from nerve pain that does not respond adequately to standard analgesics.

MECHANISM OF ACTION

Pregabalin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding reduces calcium influx at nerve terminals, decreasing the release of excitatory neurotransmitters including glutamate, norepinephrine, and substance P. The resulting reduction in neuronal excitability produces analgesic, anticonvulsant, and anxiolytic effects. Unlike gabapentin, pregabalin has more predictable linear pharmacokinetics and faster onset of action.

DOSAGE AND ADMINISTRATION

•      Neuropathic pain: 75mg twice daily; increase to 150mg twice daily within one week if needed

•      Fibromyalgia: 75mg twice daily; increase to 150mg twice daily; maximum 225mg twice daily

•      Partial seizures (adjunct): 75mg to 150mg twice daily; maximum 300mg twice daily

•      Generalized anxiety disorder: 150mg to 600mg per day in 2 to 3 divided doses

•      Take with or without food; dose reduction required in renal impairment

SIDE EFFECTS

Common:

•      Dizziness, somnolence (most common side effects)

•      Weight gain, peripheral oedema

•      Blurred or double vision

•      Dry mouth, constipation

Less common:

•      Cognitive impairment, memory problems

•      Mood changes, euphoria — potential for misuse

•      Ataxia and incoordination

•      Respiratory depression in combination with CNS depressants

NOT RECOMMENDED IN

•      Known hypersensitivity to pregabalin or gabapentin

•      Use with caution in patients with congestive heart failure

•      Reduce dose significantly in renal impairment (based on creatinine clearance)

•      Not recommended during pregnancy or breastfeeding without specialist review

DRUG INTERACTIONS

Pregabalin has additive CNS depressant effects when combined with opioids, benzodiazepines, alcohol, and other sedatives; the risk of respiratory depression is increased. Combined use with oxycodone or lorazepam has been shown to impair cognitive and gross motor function. Pregabalin does not significantly inhibit cytochrome P450 enzymes and has no clinically relevant pharmacokinetic drug interactions. Thiazolidinediones (pioglitazone, rosiglitazone) combined with pregabalin may increase the risk of oedema and weight gain.
