INDICATION

Dimis 50/200mg Tab is a fixed-dose combination of diclofenac sodium and misoprostol used for the treatment of pain and inflammation in patients who require a non-steroidal anti-inflammatory drug but are at high risk of NSAID-induced gastric ulcers. It is indicated in rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis in high-risk patients.

MECHANISM OF ACTION

Diclofenac inhibits COX-1 and COX-2, reducing prostaglandin synthesis and producing anti-inflammatory and analgesic effects. However, COX-1 inhibition reduces gastric mucosal prostaglandins, which normally protect the stomach lining. Misoprostol is a synthetic prostaglandin E1 analogue that binds to gastric mucosal receptors, stimulating mucus and bicarbonate secretion, reducing basal acid output, and promoting mucosal blood flow — directly counteracting the gastroprotective deficit caused by diclofenac.

DOSAGE AND ADMINISTRATION

•      One tablet two to three times daily with meals

•      Always take with food to reduce GI side effects from both components

•      Use the lowest effective dose for the shortest duration

•      Do not use in pregnancy because misoprostol causes uterine contractions and is abortifacient

SIDE EFFECTS

Common:

•      Abdominal pain, diarrhoea (from misoprostol component)

•      Nausea, dyspepsia

•      Headache, dizziness

•      Oedema

Less common:

•      Uterine cramping in women — do not use in pregnancy

•      Peptic ulceration despite misoprostol (at high diclofenac doses)

•      Renal impairment with prolonged use

•      Cardiovascular events with chronic NSAID use

NOT RECOMMENDED IN

•      Pregnancy (misoprostol is a known abortifacient)

•      Women of childbearing potential not using effective contraception

•      Active peptic ulceration or GI bleeding

•      Known hypersensitivity to diclofenac, misoprostol, or prostaglandins

•      Severe hepatic, renal, or cardiac impairment

DRUG INTERACTIONS

Same NSAID interactions as diclofenac alone. Antacids containing magnesium may increase the diarrhoea caused by misoprostol. Misoprostol may affect the absorption of other drugs by altering GI motility.
