INDICATION
Diclomin Eye Drop is indicated for the inhibition of intraoperative miosis during cataract surgery, relief of post-operative inflammation and pain following cataract surgery, and treatment of inflammation and pain associated with corneal refractive surgery. It is also used for symptomatic relief of seasonal allergic conjunctivitis.
MECHANISM OF ACTION
Diclofenac sodium is a topical non-steroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX-1 and COX-2) enzymes in the ocular tissues. This reduces prostaglandin synthesis, which is responsible for inflammation, pain, and miosis during intraocular procedures. By inhibiting prostaglandin-mediated vasodilation and vascular permeability, diclofenac reduces post-operative ocular inflammation and prevents surgically induced intraoperative miosis.
DOSAGE AND ADMINISTRATION
· For post-operative inflammation: instill 1 drop in the affected eye four times daily starting 24 hours after surgery.
· Treatment may continue for up to 28 days.
· For pre-operative miosis prevention: instill 1 drop four times in the 2 hours prior to surgery.
· Tilt head back, pull lower lid down, and instill the drop.
· Keep eyes closed for 1 to 2 minutes after instillation.
SIDE EFFECTS
Common:
• Mild transient burning or stinging on instillation
• Temporary blurring of vision
Less common:
• Corneal epithelial defects with prolonged use
• Increased bleeding tendency in patients on anticoagulants
• Allergic conjunctivitis in sensitive individuals
NOT RECOMMENDED IN
• Patients with known hypersensitivity to diclofenac, aspirin, or other NSAIDs
• Patients with aspirin-sensitive asthma
• Soft contact lens wearers during treatment
DRUG INTERACTIONS
• Concurrent systemic NSAIDs or anticoagulants may increase bleeding risk
• Topical corticosteroids used together should be monitored for additive immunosuppressive effects
• Other ophthalmic preparations should be applied at least 5 minutes apart
