INDICATION

Diajard-M XR 12.5/1000mg is an extended-release combination tablet indicated as an adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus. The extended-release formulation of metformin provides better gastrointestinal tolerability compared to the immediate-release combination.

MECHANISM OF ACTION

Empagliflozin inhibits SGLT2 receptors in the renal proximal tubule, increasing urinary glucose excretion and lowering blood glucose and body weight. Metformin HCl extended-release delivers the drug slowly over 24 hours, reducing peak plasma concentration and minimizing gastrointestinal side effects. Its primary mechanism remains activation of AMPK with reduction of hepatic glucose production and improved insulin sensitivity. The combination addresses multiple glycemic pathways with a favorable tolerability profile.

DOSAGE AND ADMINISTRATION

·        One tablet orally once daily with the evening meal to maximize absorption and minimize gastrointestinal side effects.

·        Swallow whole; do not crush, split, or chew.

·        Dose individualization based on renal function, current antidiabetic regimen, and blood glucose response.

·        Monitor renal function regularly.

SIDE EFFECTS

Common:

•      Gastrointestinal effects including nausea and diarrhea at initiation (less than immediate-release)

•      Urinary tract or genital infections

Less common:

•      Hypoglycemia in combination with other antidiabetics

•      Rare cases of lactic acidosis with metformin in renal impairment

•      Volume depletion with SGLT2 inhibitor component
