INDICATION

Diajard-M 12.5/1000mg is a dual oral antidiabetic combination indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus who are not adequately controlled on metformin alone or already on both empagliflozin and metformin as separate tablets.

MECHANISM OF ACTION

Empagliflozin is a sodium-glucose cotransporter 2 (SGLT2) inhibitor that reduces blood glucose by blocking glucose reabsorption in the renal proximal tubule, leading to glucosuria. This mechanism is insulin-independent. Metformin reduces hepatic glucose output via activation of AMP-activated protein kinase (AMPK) and improves peripheral insulin sensitivity. The combination provides complementary glycemic control with the added cardiovascular and renal benefits associated with SGLT2 inhibitor therapy.

DOSAGE AND ADMINISTRATION

·        One tablet orally twice daily with meals or as directed by the physician. Swallow whole without crushing or chewing.

·        The dose is individualized based on current antidiabetic therapy, renal function, and glycemic response.

·        Regular monitoring of renal function and blood glucose is required.

SIDE EFFECTS

Common:

•      Nausea, vomiting, or diarrhea, particularly at initiation

•      Urinary tract infections

•      Genital mycotic infections

Less common:

•      Volume depletion or hypotension, particularly in the elderly

•      Hypoglycemia when combined with insulin or sulphonylureas

•      Increased urinary frequency

NOT RECOMMENDED IN

•      Renal impairment with eGFR below 30 ml/min/1.73m2

•      Type 1 diabetes or diabetic ketoacidosis

•      Hypersensitivity to empagliflozin or metformin

•      Radiologic contrast requiring iodine within 48 hours

DRUG INTERACTIONS

•      Diuretics may increase risk of dehydration with empagliflozin component

•      Cimetidine may increase metformin levels
