INDICATION

Dexokyt 60mg Cap is used for the healing of all grades of erosive esophagitis (grades A through D) in adults. The 60mg dose achieves the degree of acid suppression needed for mucosal healing of erosive or ulcerated esophageal tissue. After confirmed healing, patients are typically stepped down to Dexokyt 30mg once daily for maintenance.

MECHANISM OF ACTION

Same dual delayed-release mechanism as Dexokyt 30mg. At 60mg, the two-peak plasma profile delivers a higher area under the curve (AUC), producing more complete inhibition of H+/K+-ATPase across the parietal cell population throughout the day. This results in intragastric pH remaining above 4 for a greater proportion of the 24-hour period — the threshold required for reliable healing of erosive esophagitis. Unlike older single-release PPIs that require pre-meal dosing to activate pump-bound dexlansoprazole, the DDR technology provides flexible dosing independent of meal timing while still achieving therapeutic acid suppression.

DOSAGE AND ADMINISTRATION

•      Healing of erosive esophagitis: 60mg once daily for up to 8 weeks

•      Take at any time of day with or without food

•      Capsule may be opened and granules sprinkled on applesauce and swallowed immediately; do not chew granules

•      After healing confirmed, step down to 30mg once daily for maintenance

•      In moderate hepatic impairment: reduce dose to 30mg once daily; avoid in severe hepatic impairment

SIDE EFFECTS

Common:

•      Diarrhoea, abdominal pain, nausea

•      Flatulence, vomiting

•      Headache

•      Upper respiratory tract infections

Less common:

•      Hypomagnesaemia — monitor magnesium levels with prolonged use

•      Vitamin B12 malabsorption with long-term therapy

•      Clostridium difficile-associated diarrhoea

•      Bone fractures with prolonged high-dose use

•      Acute tubulointerstitial nephritis (rare)

•      Severe cutaneous adverse reactions including Stevens-Johnson syndrome (rare)

NOT RECOMMENDED IN

•      Known hypersensitivity to dexlansoprazole, lansoprazole, or any PPI

•      Concurrent rilpivirine-containing antiretroviral regimens — absolute contraindication

•      Severe hepatic impairment

•      Rule out gastric malignancy before initiating prolonged acid suppression therapy

DRUG INTERACTIONS

Same as Dexokyt 30mg. At the 60mg dose, CYP2C19 inhibitory effects and the impact on pH-dependent drug absorption may be more pronounced. Warfarin INR, digoxin plasma levels, and signs of methotrexate toxicity should be monitored more closely at this higher dose. Concomitant rilpivirine is absolutely contraindicated. Sucralfate and antacids should be separated by at least 30 minutes.
