INDICATION

Dexokyt 30mg Cap is a proton pump inhibitor with a dual delayed-release formulation used for the maintenance of healed erosive esophagitis and for the relief of heartburn associated with symptomatic non-erosive GERD. The 30mg dose is the standard maintenance dose used after erosive esophagitis has been healed with the 60mg strength, and for ongoing heartburn control in non-erosive GERD.

MECHANISM OF ACTION

Dexlansoprazole is the R-enantiomer of lansoprazole. It is a prodrug activated in the acidic environment of gastric parietal cell secretory canaliculi, where it covalently and irreversibly binds to cysteine residues on the H+/K+-ATPase proton pump, blocking acid secretion. Its unique dual delayed-release (DDR) capsule contains two types of granules with coatings that dissolve at different pH levels. The first granule type releases dexlansoprazole at pH 5.5 in the proximal small intestine (producing a plasma peak at 1 to 2 hours), and the second releases it at pH 6.75 in the more distal small intestine (producing a second peak at 4 to 5 hours). This two-peak plasma profile extends the duration of acid suppression beyond a single-release PPI, providing effective pH control throughout the day including late-day and nocturnal acid periods — without requiring the patient to time doses before meals.

DOSAGE AND ADMINISTRATION

•      Maintenance of healed erosive esophagitis: 30mg once daily for up to 6 months

•      Symptomatic non-erosive GERD: 30mg once daily for 4 weeks

•      Can be taken at any time of day, with or without food

•      Capsule may be opened and granules sprinkled on a tablespoon of applesauce and swallowed immediately without chewing

•      No dose adjustment required in mild to moderate hepatic impairment; avoid in severe hepatic impairment

SIDE EFFECTS

Common:

•      Diarrhoea, abdominal pain

•      Nausea, flatulence, vomiting

•      Headache, dizziness

•      Upper respiratory tract infection

Less common:

•      Hypomagnesaemia with prolonged use — may cause muscle cramps, tetany, arrhythmias

•      Vitamin B12 deficiency with long-term therapy

•      Clostridium difficile-associated diarrhoea

•      Osteoporosis-related bone fractures with prolonged use

•      Acute tubulointerstitial nephritis (rare)

•      Cutaneous and systemic lupus erythematosus (rare)

NOT RECOMMENDED IN

•      Known hypersensitivity to dexlansoprazole, lansoprazole, or any other PPI

•      Concurrent use with rilpivirine-containing HIV regimens — absolute contraindication

•      Severe hepatic impairment

•      Symptomatic response does not rule out gastric malignancy — investigate before starting prolonged therapy

DRUG INTERACTIONS

Dexlansoprazole reduces the absorption of drugs that require an acidic gastric environment, including ketoconazole, itraconazole, iron salts, erlotinib, and atazanavir. As a moderate CYP2C19 inhibitor, it may increase plasma levels of warfarin (monitor INR), diazepam, and phenytoin. Concurrent use with clopidogrel may reduce the antiplatelet activity of its active metabolite. It can increase plasma concentrations of digoxin, methotrexate, and tacrolimus — monitor appropriately. Sucralfate and antacids reduce dexlansoprazole absorption; administer dexlansoprazole at least 30 minutes before sucralfate.
