INDICATION

Dexiva 60mg Cap is a proton pump inhibitor (PPI) with a dual delayed-release formulation, used for the healing and maintenance of erosive esophagitis, symptomatic non-erosive gastroesophageal reflux disease (GERD), and heartburn relief. The 60mg dose is used for the acute healing phase of erosive esophagitis.

MECHANISM OF ACTION

Dexlansoprazole is the R-enantiomer of lansoprazole. It is a prodrug that requires activation in the acidic parietal cell secretory canaliculi. Once activated, it covalently binds to the H+/K+-ATPase proton pump, irreversibly inhibiting acid secretion. The dual delayed-release system delivers dexlansoprazole in two separate pulses, extending acid suppression beyond 24 hours with a single daily dose.

DOSAGE AND ADMINISTRATION

•      Healing of erosive esophagitis: 60mg once daily for 8 weeks

•      Maintenance of healed erosive esophagitis: 30mg once daily

•      Symptomatic GERD: 30mg once daily for 4 weeks

•      Can be taken with or without food at any time of day (unlike older PPIs)

•      Capsule may be opened and the granules sprinkled on a tablespoon of applesauce for patients with difficulty swallowing

SIDE EFFECTS

Common:

•      Diarrhea, abdominal pain

•      Nausea, flatulence

•      Headache, dizziness

Less common:

•      Hypomagnesaemia with prolonged use

•      Vitamin B12 deficiency with long-term therapy

•      Clostridium difficile-associated diarrhea

•      Osteoporosis-related bone fractures with long-term high-dose use

NOT RECOMMENDED IN

•      Known hypersensitivity to dexlansoprazole, lansoprazole, or other PPIs

•      Concurrent use with rilpivirine-containing HIV regimens

•      Use with caution in patients with severe hepatic impairment

DRUG INTERACTIONS

Dexlansoprazole reduces absorption of drugs requiring an acidic gastric environment, including ketoconazole, itraconazole, iron salts, erlotinib, and atazanavir. It can increase plasma levels of digoxin, methotrexate, and tacrolimus.
