INDICATION

Dayline 500mg IM Inj is a ceftriaxone injection formulated for intramuscular administration. It is used for the treatment of moderate bacterial infections in patients who require parenteral therapy but where IV access is not available or appropriate. Common indications include community-acquired pneumonia, pyelonephritis, skin infections, gonorrhea, and use as step-down therapy from IV ceftriaxone.

MECHANISM OF ACTION

Ceftriaxone binds to penicillin-binding proteins and inhibits bacterial cell wall synthesis, causing bactericidal cell death. The IM formulation is often prepared with 1% lidocaine to reduce injection site pain.

DOSAGE AND ADMINISTRATION

•      Adults: 500mg to 1g IM once daily for most indications

•      Reconstitute with 1% lidocaine solution for IM administration to reduce pain

•      Use the gluteal muscle or lateral thigh for IM injection; avoid the same site repeatedly

•      Gonorrhea: 500mg IM as a single dose

•      Rotate injection sites; confirm needle placement before administering

SIDE EFFECTS

Common:

•      Pain and induration at the injection site

•      Diarrhea, nausea

•      Skin rash

Less common:

•      Biliary sludge with prolonged courses

•      Allergic reactions

•      Hematological changes

NOT RECOMMENDED IN

•      Known allergy to cephalosporins or penicillin’s

•      Neonates with hyperbilirubinemia

•      Do not mix with calcium-containing solutions

•      IM route contraindicated in patients with coagulopathy or very low platelet count

DRUG INTERACTIONS

If lidocaine is used for reconstitution, apply lidocaine precautions in patients with known lidocaine sensitivity or cardiac conduction disorders.
