INDICATION
Dayline 1g IV Inj is a third-generation cephalosporin antibiotic used for the treatment of serious bacterial infections including community-acquired pneumonia, hospital-acquired infections, meningitis, septicemia, intra-abdominal infections, pelvic inflammatory disease, urinary tract infections, skin and soft tissue infections, bone and joint infections, and gonorrhea. It is also used for perioperative surgical prophylaxis.
MECHANISM OF ACTION
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), specifically PBPs 1 and 3. This prevents the cross-linking of peptidoglycan chains, disrupting the structural integrity of the bacterial cell wall and causing osmotic lysis and cell death. As a third-generation cephalosporin, it has enhanced stability to many beta-lactamase enzymes and broader Gram-negative coverage than earlier cephalosporins.
DOSAGE AND ADMINISTRATION
• Moderate to severe infections: 1g to 2g IV once daily
• Meningitis: 2g IV twice daily for 10 to 14 days
• Gonorrhea: 250mg to 500mg IM or IV as a single dose
• Surgical prophylaxis: 1g IV 30 to 60 minutes before the procedure
• Reconstitute with compatible diluent; administer by slow IV infusion over 30 minutes or IV push over at least 4 minutes
SIDE EFFECTS
Common:
• Injection site reactions (pain, redness, phlebitis)
• Diarrhea, nausea
• Rash
• Elevated liver enzymes
Less common:
• Clostridium difficile-associated colitis
• Biliary sludge or gallstones with prolonged use
• Hemolytic anemia, eosinophilia, thrombocytopenia
• Hypersensitivity reactions including anaphylaxis in penicillin-sensitive patients
NOT RECOMMENDED IN
• Known hypersensitivity to ceftriaxone, cephalosporins, or penicillin’s (10% cross-reactivity)
• Neonates with jaundice or hyperbilirubinemia
• Physically incompatible with calcium-containing IV solutions
• Hypocalcemia in premature neonates
DRUG INTERACTIONS
Ceftriaxone must never be mixed or co-administered with calcium-containing solutions, as it forms a potentially fatal precipitate in neonates. Concurrent use with aminoglycosides provides synergistic activity against Gram-negative bacteria. Anticoagulant effect of warfarin may be enhanced; monitor INR.
STORAGE INSTRUCTIONS
Store Dayline 1g Inj powder vials at room temperature below 25°C. Protect from light. After reconstitution, use within 6 hours at room temperature or 24 hours when refrigerated. Keep out of reach of children.
