INDICATION

Dapakan Syrup 250mg/5ml is an anticonvulsant and mood-stabilizing agent in syrup form indicated for the treatment of all forms of epilepsy including absence seizures, myoclonic epilepsy, tonic-clonic seizures, partial seizures, and Lennox-Gastaut syndrome. It is also indicated for the prophylaxis of migraine headaches and the treatment of manic episodes associated with bipolar disorder. The syrup formulation is particularly suitable for children and adults unable to swallow tablets or capsules.

MECHANISM OF ACTION

Valproic acid is a branched short-chain fatty acid with multiple complementary anticonvulsant mechanisms. It increases brain GABA concentrations by inhibiting GABA-transaminase and succinic semialdehyde dehydrogenase enzymes, thereby reducing GABA breakdown and increasing its inhibitory tone in the brain. It also blocks voltage-gated sodium channels in their inactivated state, reducing the rapid repetitive firing of neurons. Additionally, it inhibits T-type calcium channels in thalamic neurons, which is particularly important in abolishing absence seizures. In bipolar disorder, valproic acid modulates signal transduction pathways including PKC and inositol phosphate signaling.

DOSAGE AND ADMINISTRATION

•      Epilepsy in children: initially 10 to 15mg/kg/day in divided doses; increase by 5 to 10mg/kg/week to a maintenance dose of 20 to 30mg/kg/day

•      Adults: 600mg/day initially in 2 to 3 divided doses; titrate to 1000 to 2000mg/day

•      Take with food to reduce gastrointestinal side effects

•      Never stop abruptly; always follow the titration and monitoring schedule prescribed by your neurologist; liver function and blood counts monitored regularly

SIDE EFFECTS

Common:

•      Nausea, vomiting, and gastrointestinal discomfort (take with food)

•      Weight gain (very common with long-term use)

•      Tremor (fine hand tremor; dose-dependent)

•      Alopecia (hair loss; often temporary; consider zinc/selenium supplementation)

Less common:

•      Hepatotoxicity: elevated liver enzymes and fatal hepatic failure (monitor LFTs; highest risk in children under 2 years)

•      Pancreatitis (potentially fatal; stop if severe abdominal pain develops)

•      Hyperammonemia and encephalopathy (monitor ammonia in confused patients)

•      Teratogenicity: valproic acid causes major congenital malformations and developmental delay; strictly contraindicated in pregnancy

NOT RECOMMENDED IN

•      Pregnancy (highest teratogenic risk of any antiepileptic; causes neural tube defects and neurodevelopmental delay; absolutely avoid in women of childbearing potential unless no alternative)

•      Personal or family history of severe hepatic disease

•      Mitochondrial disorders caused by POLG mutations

•      Known urea cycle disorders

DRUG INTERACTIONS

Carbapenem antibiotics (imipenem, meropenem) dramatically reduce valproic acid levels and can precipitate seizures; avoid combination. Lamotrigine levels are markedly increased by valproic acid; halve the lamotrigine dose when combined. Phenytoin, carbamazepine, and phenobarbitone increase valproic acid clearance, requiring higher doses. Aspirin displaces valproic acid from plasma proteins, transiently increasing free drug levels.
