INDICATION

Dapa 5mg Tab is a sodium-glucose cotransporter-2 (SGLT2) inhibitor prescribed for the management of type 2 diabetes mellitus in conjunction with diet and exercise. It is also used to reduce the risk of cardiovascular death and hospitalization for heart failure in patients with type 2 diabetes and established cardiovascular disease, and to slow the progression of chronic kidney disease.

MECHANISM OF ACTION

Dapagliflozin selectively and reversibly inhibits sodium-glucose cotransporter-2 (SGLT2) in the proximal renal tubule. SGLT2 is responsible for approximately 90% of renal glucose reabsorption. By blocking this transporter, dapagliflozin causes excess glucose to be excreted in the urine (glucosuria), lowering blood glucose independently of insulin. This mechanism also induces a mild osmotic diuresis and natriuresis, contributing to reductions in blood pressure, plasma volume, and body weight.

DOSAGE AND ADMINISTRATION

•      Type 2 diabetes: 5mg once daily in the morning, with or without food; may be increased to 10mg if better glycemic control is needed

•      Heart failure and CKD: 10mg once daily (higher dose preferred for these indications)

•      Patients should be adequately hydrated before initiating therapy

•      Not recommended when eGFR is below 25ml/min/1.73m2 for diabetes indication

SIDE EFFECTS

Common:

•      Genital mycotic infections (yeast infections) in both men and women

•      Urinary tract infections

•      Increased urination (polyuria)

•      Nasopharyngitis

Less common:

•      Diabetic ketoacidosis (even with near-normal blood glucose levels)

•      Acute kidney injury, especially with concurrent dehydration

•      Hypotension in patients on diuretics or antihypertensives

NOT RECOMMENDED IN

•      Type 1 diabetes mellitus

•      Diabetic ketoacidosis

•      Severe renal impairment (eGFR below 25ml/min/1.73m2)

•      Known hypersensitivity to dapagliflozin

•      Pregnancy and breastfeeding

DRUG INTERACTIONS

Dapagliflozin combined with insulin or sulphonylureas increases the risk of hypoglycemia; dose reduction of the insulin or sulphonylureas may be needed. Diuretics combined with dapagliflozin increase the risk of dehydration and hypotension. NSAIDs and ACE inhibitors used concurrently may increase the risk of acute kidney injury.

STORAGE INSTRUCTIONS

Store Dapa 5mg tablets at room temperature between 15°C and 30°C. Protect from moisture and heat. Keep in original blister packaging. Store out of reach of children.
