INDICATION

Danzol 200mg Cap is a synthetic steroid used for the treatment of endometriosis, fibrocystic breast disease, and hereditary angioedema. In endometriosis, it suppresses the ovarian-pituitary axis and creates a hypoestrogenic state that causes atrophy and regression of endometrial implants. In fibrocystic breast disease, it reduces breast pain, tenderness, and nodularity.

MECHANISM OF ACTION

Danazol inhibits the pulsatile release of gonadotropin-releasing hormone (GnRH) and gonadotropins (LH and FSH), suppressing the mid-cycle LH surge and ovulation. This creates a hypoestrogenic, hypo progestogenic environment that causes endometrial atrophy and regression of ectopic endometrial tissue. Danazol also binds to androgen, progesterone, and glucocorticoid receptors and may directly inhibit steroidogenesis. In hereditary angioedema, it stimulates the hepatic synthesis of C1-esterase inhibitor.

DOSAGE AND ADMINISTRATION

•      Endometriosis (mild to moderate): 200mg to 400mg twice daily for 3 to 6 months

•      Severe endometriosis: 800mg/day in divided doses

•      Fibrocystic breast disease: 100mg to 200mg twice daily for 3 to 6 months

•      Hereditary angioedema: 200mg two to three times daily; reduce to lowest effective dose

•      Start treatment during menstruation to exclude pregnancy

SIDE EFFECTS

Common:

•      Weight gain, acne, oily skin

•      Menstrual irregularities or amenorrhea

•      Hirsutism (increased facial/body hair)

•      Hot flushes due to estrogen suppression

Less common:

•      Voice deepening (virilization) but it may be irreversible

•      Clitoromegaly

•      Hepatotoxicity and elevated liver enzymes with long-term use

•      Thromboembolic events

NOT RECOMMENDED IN

•      Pregnancy and breastfeeding (teratogenic and virilizing to female fetus)

•      Active liver disease or hepatic dysfunction

•      Undiagnosed genital bleeding

•      Androgen-dependent tumor

•      Thromboembolic history or porphyria

DRUG INTERACTIONS

Danazol inhibits CYP3A4 and can significantly increase plasma levels of cyclosporine, tacrolimus, carbamazepine, and warfarin; monitor these drugs closely. It can potentiate the anticoagulant effect of warfarin, increasing bleeding risk. Danazol may affect insulin sensitivity; monitor blood glucose in diabetic patients.
